摘  要:目的研究T2DM患者给予极低热量限制(VLCR)治疗前后血浆代谢物的变化,从代谢组学角度阐述短期VLCR改善T2DM糖脂代谢的作用机制。方法采用超高效液相色谱串联质谱技术采集VLCR治疗前后10例T2DM患者的血浆代谢物谱数据。采用偏最小二乘判别分析(PLS–DA)和正交偏最小二乘判别分析(OPLS–DA)模型对治疗前后血浆代谢物进行对比分析。结果采用PLS–DA法可成功区分T2DM患者VLCR前组与热量限制后组,采用OPLS–DA法发现了5个潜在的生物标记物,溶血磷脂酰胆碱(16:0)、溶血磷脂酰胆碱[18:2(9 z,12 z)]、溶血磷脂酰胆碱[20:4(7 z,8 z,11 z,14 z)]、戊酰肉碱、亮氨酸,并针对5个生物标记物对VLCR治疗可能的作用机制进行讨论。结论 T2DM患者VLCR治疗前后血浆代谢物比较,差异有统计学意义。VLCR可调节T2DM患者体内代谢紊乱,其作用机制可能与升高溶血磷脂酰胆碱(16:0)、溶血磷脂酰胆碱[18:2(9 z,12 z)]、溶血磷脂酰胆碱[(20:4(7 z,8 z,11,14 z)]、戊酰肉碱、亮氨酸有关。Objective To investigate the changes of plasma metabolome in patients with type 2 dia-betes mellitus(T2 DM)after very low calorie restriction(VLCR)therapy,and to explore the underlying mechanism of glycolipid improvement effect by calorie restriction in patients with T2 DM by metabonomics.Methods Plasma metabolite data before and after VLCR was collected by Ultra-performance Liquid Chro-matography Tandem Mass Spectrometry in 10 diabetic patients. Plasma metabolome before and after VLCR was compared by partial least square analysis of variance(PLS–DA)and orthogonal partial least squares-dis-criminate analysis(OPLS–DA)models.Results The PLS–DA method could successfully distinguish the pre-caloric restriction group and the post-caloric restriction group in patients with T2 DM. Five potential biomarkers,lysophosphatidylcholine(16:0),lysophosphatidylcholine(18:2(9 z,12 z)),lysophosphatidylcho-line [20:4(7 z,8 z,11 z,14 z)],valerylcarnitine,and leucine were identified using the OPLS-DA method.Conclusion There was a significant difference between the plasma metabolome before and afterVLCR ther-apy. VLCR may regulate metabolic disturbances in type 2 diabetic patients by elevating lysophosphatidylcho-line(16:0),lysophosphatidylcholine [(18:2(9 z,12 z)],lysophosphatidylcholine [(20:4(7 z,8 z,11 z,14 z)],valerylcarnitine,and leucine.
