多普勒超声心动图在移植心脏中的应用

摘　　要：目的　通过对移植心脏的观察 ,了解移植心脏多普勒超声心动图的特征。方法　仪器为美国U ltrasound— 9型超声显像仪 ,以多个标准及非标准切面观察、计算移植心脏的多个指标。结果　 2 D示右室壁进行性肥厚 ,6 0 d时达最厚 ;左室主要表现为间隔运动异常 ;心包积液少量 ,4个月后消失 ,左胸腔积液少量 ,2 7d后消失。多普勒超声表现 :E峰速度为 (0 .72± 0 .0 7) m /s,A峰速度为 (0 .4± 0 .0 4) m /s,E/A为 1.8± 0 .4,IVRT为(80± 11) m/s,PHT为 (6 6± 7) ms。结论　利用多普勒超声心动图观察移植心脏的多个指标 ,能够正确评价移植心脏的形态功能 ,及有无排异反应。Objective To study the characteristics of Doppler ultrasound cardiogram of the heart transplanted.Method The heart transplanted was scanned by ultrasound type 9 ultrasonic diagnostic apparatus.Both standard and nonstandard sections were scanned and analysed.The ultrasonic parameters of the heart transplanted were calculated.Results Two dimensional ultrasound scan showed progressive thickening of right ventriculur wall,which peaked 60 days later;abnormal movement of ventricular septum;small quantity of hydro pericardium,which disappeared 4 months later;a little hydrothorax on left side,which disappeared 27 days later.Doppler ultrasound scan showed velocity of E peak to be (0.72±0.07)m/s,velocity of A peak (0.4±0.04)m/s,ratio of E/A 1.8±0.4,IVRT (80±11)m/s and PHT (66±7)ms.Conclusion With Doppler ultrasound cardiogram,we can evaluate the morphology and function of the heart transplanted and judge if immunological rejection has occurred.
