摘  要:目的 :研究自发性高血压大鼠 (SHR)动脉压力感受性反射 (ABR)功能的改变及其与心、肾、血管等靶器官损伤(TOD)间的关系。 方法 :分别测定清醒、自由活动状态下 SHR及 WKY大鼠 2 4 h动脉血压、心率、血压波动性 (BPV)及心率波动性 (HRV) ;测定 ABR对心率及对血压的控制功能 (ABR- HP,ABR- BP) ;按预先制定的评分标准对高血压 TOD进行评估。 结果 :SHR的 2 4 h收缩压 (SBP)、舒张压 (DBP)及收缩压波动性 (SBPV)、舒张压波动性 (DBPV)显著高于对照 WKY大鼠 ,心率二者间无差异 ,但 SHR的 HRV显著低于 WKY大鼠。 SHR的 ABR功能显著低于 WKY大鼠 (P<0 .0 1)。 SHR靶器官损伤明显 ,TOD评分显著高于对照的 WKY大鼠。直线相关分析结果表明 :SHR的 TOD得分与 SBP及 DBP呈正相关 (P<0 .0 5 ) ;与 BPV也呈正相关 (P<0 .0 1) ;与 ABR功能呈负相关 (P<0 .0 1) ;TOD得分与心率间无相关关系 ,但与心率波动性间呈负相关 (P<0 .0 5 )。 TOD得分与 ABR功能间相关系数高于与血压、BPV等其他血流动力学变量间相关系数。多元逐步回归分析结果进一步表明 :在血压、心率、BPV、HRV及 ABR功能等一系列变量中 ,ABR功能对 SHR的靶器官损伤影响作用最强 ,血压水平次之。 结论 :ABR功能与高血压 TOD密切相关 。Objective:To investigate the correlation between arterial baroreceptor reflex (ABR) function and target organ dam age (TOD) in spontaneously hypertensive rats (SHR) .Methods:Twenty- four- hour blood pressure (SBP and DBP ) ,blood pressure variability (BPV ) ,heart rate (HR ) and HR variability (HRV ) were m easured in conscious, unrestrained SHR and Wistar- Kyoto (WKY ) rats.ABR function control of heart period (ABR- HP) and blood pressure (ABR- BP) were determined respectively.Hypertensive TOD was evaluated according to the scoring system.Results:SBP, DBP and their BPV were significantly increased in SHR compared with those of WKY rats.No difference of HR was found between the 2 strains,but HRV was significantly decreased in SHR when com pared with WKY rats.ABR- HP and ABR- BP of SHR were significantly decreased compared with those of WKY rats (P <0 .0 1) .SHR had remarkable TOD when com pared with WKY rats (TOD score:6 .3± 2 .5 vs 2 .9± 0 .8,P<0 .0 1) .L inear regressive analysis dem onstrated that TOD score was positively related to SBP and DBP (P<0 .0 5 ) .Also TOD score was positively related to BPV (P<0 .0 1) .In SHR, TOD score was negatively related to ABR function (P<0 .0 1) .No significant correlation was found between TOD score and HR,but TOD score was negatively related to HRV (P<0 .0 5 ) .Stepwise m ultiple regressive analysis showed that TOD score was predicted by greater SBP and by lower ABR function and lower HRV .The contribution of ABR function to hypertensive TOD was the greatest am ong the above hemodynamic variables.Conclusion:Arterial baroreflex plays an important role in hypertensive TOD,and impaired ABR function may aggravate TOD in hypertension. [
