摘　　要：目的　探讨圆锥动脉干畸形 (CTD)的经胸三维超声 (3DTTE)诊断价值并确定有意义的 3D图像。方法　用HP 5 5 0 0型超声仪和TOMTEC工作站对CTD作 3DTTE检查 ,与心血管造影或手术对照。结果　 3 2 /3 6例 (88.89% )三维重构成功 ,纠正二维超声诊断 6/3 2例 (18.75 % ) ,经造影证实。其中 16例手术 ,3D诊断与手术一致。并确定心脏四腔观(H) ,房、室间隔左心或右心观 (L1a或L2a) ,瓣膜俯视或仰视观 (S1a或S2a)等剖视面对CTD有诊断意义。结论　 3DTTE能对CTD作出较准确的三维空间诊断。Objective To evaluate the diagnostic value of transthoracic dynamic three dimensional echocardiography (3DTTE) and attempts to establish a series of useful and helpful imaging views for 3DE sectional diagnoses of conotruncal defects (CTD) in children. Methods Thirty six cases of children with CTD were selected for 3DTTE study by HP SONOS 5500 echocardiographic system interfaced with TOMTEC image system. They were examined with dynamic 3DTTE, 2DE and Angiocardiography, respectively. Some of them were compared with the results of operations, served as 'Gold Standard'. Results Three dimensional reconstruction was possible in 32/36 cases (88.89%). In 6 cases (18.75%), 2DE diagnosis was corrected by 3DE and conformed angiocardiography. The diagnosis of 3DE complied with the operation findings in the 16 cases received operations. All cut planes and their section views reconstructed by 3DTTE could display almost important inner cardiac structures. Especially, a series of useful and helpful 3D section views were identified for CTD diagnoses as following:①Whole heart oblique frontal plane (H); ②En face view of atrial (ventricular) septum from left ventricle (L1a) or from right ventricle (L2a);③En face view of atrial ventricular valve from atria (S1a) or from ventricles (S2a). Conclusion The identified images of 3DTTE section views could be demonstrated detail spatial information of CTD. Thus, 3DTTE will be a reliable diagnostic method and will play an important role in diagnosing CTD.
