摘  要:目的 探讨中、晚期孕妇TORCH系列病原体感染对母儿的影响。②方法 采用聚合酶链反应(PCR)对 2 2 5例中、晚期孕妇进行弓形虫 (TOX)、沙眼衣原体 (CT)、解脲支原体 (UU)、风疹病毒 (RUV)、巨细胞病毒 (CMV)、Ⅱ型单纯疱疹病毒 (HSV Ⅱ )检测。随访 78例病原体阳性孕妇 (阳性组 )和 92例病原体阴性孕妇 (阴性组 ) ,比较其妊娠结局。③结果  2 2 5例中、晚期孕妇上述病原体总感染率为 36 % (81/ 2 2 5 ) .孕妇上述病原体感染可导致宫内感染。阳性组死胎、胎儿宫内窘迫、胎膜早破、新生儿重度窒息发生率显著高于阴性组 (χ2 =4 .6 4 ,6 .0 1,6 .0 8,P <0 .0 5 )。其中混合病原体感染组胎膜早破、胎儿宫内窘迫、新生儿重度窒息发生率显著高于单一病原体感染组 (χ2 =4 .77,11.35 ,3.89,P <0 .0 5 )。④结论 孕期TORCH感染对母儿均可产生不良影响 ,应重视对混合病原体感染孕妇的早期筛查和诊治。Objective\ To study the positive rates of TORCH infection during midtrimester and late pregnancy and its effect on pregnant women and fetus. \ Methods\ The polymerase chain reaction (PCR) was used to detect the pathogens, including toxoplasma, chlamydia trachomatis, ureaplasma urealyticum, rubella virus, cytomegalovirus and herpes simplex type Ⅱin 225 cases of midtrimester and late pregnancy. By following up survey of 78 cases of infection group and 92 cases of noninfection group, the pregnancy outcomes of the two groups were compared. \ Results\ The positive rates of TORCH infection was 36%(81/215) for second and third trimester of pregnancy. Higher rates of preterm rupture of membrane (PROM),fetal distress, severe asphyxia of neonate were found in infection group of TORCH ( χ 2=4.64,6.01,6.08,P <0.05). The rates of PROM, fetal distress, severe asphyxia of neonate in infection group of multiple pathogens were obviously higher than that of single pathogen ( χ 2=4.77,11.35,3.89,P <0.05). \ Conclusion\ TORCH is a group of pathogens that cause infection of pregnant women and fetus. More attention should be paid to the early diagnosis and therapy of multiple pathogens infection.
