摘 要:目的 探讨骨基质明胶 (bonematrixgelatin ,BMG)促进骨愈合的作用。 方法 95例骨折愈合障碍患者行内固定和植入BMG(BMG组 ) ,5 0例行内固定和自体植骨 (自体植骨组 )。BMG组 30例行病理切片观察 ,BMG组和自体植骨组各 2 0例于术前、术后 1周、术后 1个月检测骨钙素(osteocalcin ,BGP)和骨特异碱性磷酸酶 (bonespecificalkalinephosphatase ,BSAP) ,BMG组 2 0例检测术前、术后 1周、术后 2周的白细胞介素 2 (IL - 2 )、IL - 4、IL - 6和IL - 10。 结果 BMG组骨折愈合时间与自体植骨组差异无显著性意义 (P >0 .0 5 ) ,病理切片示诱导成骨与正常骨结构差异亦无显著性意义 (P >0 .0 5 ) ,两组术后BGP、BSAP均高于术前 (P <0 .0 1) ,术后 1个月时BMG组的BGP较自体植骨组高 (P <0 .0 5 ) ,BMG组术前与术后的IL - 2、IL - 4、IL - 6、IL - 10差异无显著性意义 (P>0 .0 5 )。 结论 BMG具有促进骨生长的作用 。Objective To investigate the effect of bone matrix gelatin (BMG) on fracture healing. Methods A total of 95 patients with disturbance in fracture healing were treated with internal fixation and BMG grafts (the BMG group), and 50 patients were treated with internal fixation and autobones (the autobone group). The pathological section was observed in 30 cases in the BMG group. Osteocalcin (BGP) and bone specific alkaline phosphatase (BSAP) were detected respectively in 20 cases of both groups before operation, at 1 week and at 1 month after operation. While interleukin 2 (IL 2), IL 4, IL 6 and IL 10 were detected respectively in 20 cases in the BMG group before operation, at 1 week and at 2 weeks after operation. Results There existed no significant difference of time of fracture healing in the BMP group and the autobone group ( P >0.05). The pathologic biopsy showed that there was no difference of the structure between the normal bone and the bone formed by BMG. The BGP and BSAP in the two groups were higher than those before operation ( P <0.01). The BGP in the BMG group was higher than that in the autobone group ( P >0.05). The IL 2, IL 4, IL 6 and IL 10 in the BMG group had no difference before operation and after operation ( P >0.05). Conclusions BMG can promote the growth of bones, substitute the autobones and induce no immunological reaction.
