摘 要:目的 :探讨胰岛素水平升高对胎盘肥胖基因表达瘦素的影响。方法 :采用放射免疫法分别检测 35例糖尿病产妇、40例正常产妇的外周血胰岛素、瘦素水平 ,同时分别检测两组的脐血瘦素水平。采用 RT- PCR和荧光定量分析法分别检测两组的胎盘肥胖基因表达的 m RNA水平。并对结果进行统计学相关分析。结果 :1糖尿病产妇外周静脉血胰岛素水平、瘦素水平均高于正常产妇 (P<0 .0 1) ,胰岛素与瘦素水平两者具有明显的相关性 (r=0 .5 5 ,P<0 .0 0 0 1) ;2两组产妇的胎盘肥胖基因表达的 m RNA水平有明显的差异 (P<0 .0 1) ;3胎盘肥胖基因表达的 m RNA水平与脐血瘦素水平有明显的相关性 (r分别为 0 .5 2、0 .48,P<0 .0 0 1) ;4两组产妇的新生儿体重的比较也有明显的差异 (P<0 .0 1)。结论 :高胰岛素对糖尿病人胎盘肥胖基因的表达有上调作用 ,它可能是糖尿病产妇的胎儿容易出现超体重儿 (巨大儿 )Objectives:To explore action of insulin for obese gene expressing in placenta in gestational diabetes.Methods: ①Leptin in maternal blood,umbilical blood and insulin in maternal blood were detected by radiommunoassay(RIA)for 35 gestational diabetes and 40 non-diabetes pregnant women.②The expressed quality of obese gene in placenta of 35 gestational diabetes and 40 non-diabetes pregnant women were detected using RT-PCR,EB-fluorescence spectra measurement.③Results were analyzed by the correlative analysis of statistics.Results: ①Levels of insulin in maternal blood in 35 gestational diabetes is averagely 13.2±0.8mIU/L(8.2~18.3mIU/L),Levels of insulin in maternal blood in 40 non-diabetes pregnant women.is averagely 10.6±0.9 mIU/L(7.8~16.4 mIU/L).There is obviously difference in two groups(P<0.01).②The mRNA of obese gene expressing in placenta is discriminating in two groups(P<0.01).③There is obviously correction between the expressed quality of obese gene in placenta and the quality of leptin in umbilical blood(gestational diabetes:r=0.53,P<0.001;non-diabetes pregnant women:r=0.48,P<0.001).④There is obviously correction between the weight of newborn in two groups(P<0.01).Conclusion:There is a upregulative action of insulin for obese gene in the placenta of gestational diabetes.It may be one of important reason of overweight fetus in gestational diabetes.
