摘 要:目的 通过对慢性肾功能不全患者肾内血流的彩色多普勒研究 ,为临床评估肾功能损害程度提供新的指标。方法 40例患者按血清肌酐水平的不同分为Ⅰ组 (氮质血症期 )和Ⅱ组 (尿毒症期 ) ,均行彩色多普勒检查 ,测量肾内叶间及弓状动脉的收缩期峰值流速 (PS)及阻力指数 (RI) ,并记录血清肌酐 (Cr)值。结果 ①Ⅱ组患者叶间及弓状动脉的RI比Ⅰ组高 ,而PS比Ⅰ组低。②两组患者叶间及弓状动脉的PS均与Cr呈负相关 ,而RI均与Cr呈正相关。③RI较高的患者 ,Cr下降缓慢 ,甚或持续在一个较高的水平。结论 彩色多普勒测定肾内叶间及弓状动脉的血流参数 ,可为慢性肾功能损害程度的判断及预测其恢复速度提供一项新的有价值的方法。Objective To provide a new parameter to evaluate clinical renal failure by the color Doppler study of intrarenal blood flow in the patients with chronic renal failure. Methods The patients were divided into group I (azotemia stage) and group Ⅱ (uremia stage).All the patients were examed with color Doppler. Peak systolic velocity (PS) and resistance index (RI) of the interlobar and arcuate arteries were measured. The blood creatinine was recorded. Results ①The RI of interlobar and arcuate arteries in groups Ⅱ was higher than in group Ⅰ, but the PS in group Ⅱ was lower than group Ⅰ. ②The PS of interlobar and arcuate arteries in two groups were negatively correlative with the blood creatinine, whereas the RI were positively correlative with the blood creatinine. ③The level of blood creatinine in the patients with higher RI declined slowly, even persisted at a high level. Conclusion Measuring the flow indexes of interlobar and arcuate arteries with color Doppler can provides a new valuable method to judge the damage degree of the chronic renal failure and predict the recovery speed of the renal function.
