18例胸内巨大肿瘤的外科治疗分析

摘　　要：目的:探讨胸内巨大肿瘤的外科治疗经验。方法:回顾分析18例胸内巨大肿瘤患者的临床资料。男10例,女8例。左胸9例,右胸9例。肿瘤重2000～3800g。结果:无院内死亡。复张性肺水肿及胸腔内出血各1例。结论:麻醉后气管、大血管受压可引起窒息、休克危险。手术切口要充分显露,术中可分块切除或控制瘤蒂后整个切除,注意术中大出血,防止发生复张性肺水肿。Objective:To explore the surgical treatment of thoracic giant tumor.Meth-ods:Eighteen(10male and8female)patients with thoracic giant tumor were reviewed.Nine patient 's tumor were in the left chest cavity and the other9in the right.The weight of the tu-mor ranged from2000to3800gram.Results:No postoperative death was observed,but only one case had pulmonary edema and one intrathoracic hemorrhage.Conclusion:Post-anesthetic compression of trachea and major vessels may lead to suffocation and shock.The operation field must be thoroughly exposed.If the tumor is too large,it may be resected lump by lump or it can be excised totally after ligation of the pedicle of the tumor.Beware of massive bleeding in operation and postoperative pulmonary edema.
