透析液钙浓度对透析性骨病及血压的影响

摘　　要：目的 :对12例慢性血液透析患者用不同浓度钙透析液透析治疗 ,观察对其骨病及血压的影响。方法 :检验了SBUN、SCr.、SCa、SP、SAKP、SCaP、指骨X片、临床症状、透析中生理指标等。结果 :1.75～2.0mmol/LCa浓度可以使血钙维持在正常或略高水平 ,透析前SCa(2.090±0.12)mmol/L ,透析后SCa(2.390±0.32)mmol/L(P<0.01),透析中痛性痉挛消失 ,提高了对高脱水量的耐受性 ,但血压在透析第三小时偏高。结论 :视个体情况提倡“透析液钙处方” ,对暂时未服用钙三醇的患者应使用Ca浓度为1.75～2.0mmol/L的透析液。对顽固性高血压患者应使用钙离子拮抗剂或使用低钙透析液。Objective: To investigate effects of the different calcium concentration of dialysate on the dialysis bone disease and blood pressure. Methods: Serum BUN?Cr?P? AKP?CaP?finger X-ray of 12 hemodialysis patients were measured and the clinical signs were examined. Results: The dialysate of Ca 2+1.75-2.0mmol/L may keep normal concentration of Ca2+ or moderate increase of the serum free Ca2+(pre-HD is 2.09±0.12mmol/L, post-HD is 2.39±0.32 mmol/L, P<0.01).The pain of the muscle and the convulsion were disappeared and tolerance of over high ultrafiltiation for body fluid with this dialysate increase. But the blood pressure of patients were incresed at 2th?3th hour during hemodialysis.Conclustion: Recommend'prescription of dialysate Calcium' for some chronic HD patients.The dialysate of Ca2+ 1.75 -2.0mmol/L is used for patients without taking 'Rocaltrol' and Calcium channel blocker is used for HD patients of hypertention.
