早产儿应用IVIG对IgG水平及婴儿喂养情况的影响

摘  要:目的 :探讨早产儿应用静脉丙种球蛋白 (IVIG)后血清免疫球蛋白 (IgG)水平变化及对早产儿喂养情况的影响。方法 :将40例早产儿随机分为两组 :IVIG组A组及对照组B组。生后48h内给予A组IVIG400mg/kg,分别测定用药前及用药后7天两组IgG水平 ,同时观察两组患儿与喂养有关情况。结果 :用药前两组IgG水平无差别 ,用药后A组IgG水平提高 (t=2.85,P<0.05)。A、B两组在胎龄、性别、体重、开始喂养时间、开始喂养热卡数、生理性体重下降天数、排便情况无统计学差异 ,但在用药后7天喂养热卡数 (分别为61.2Kcal/kg及47.7Kcal/kg)、停止生理性体重下降后7天内体重增长克数 (分别为10.9g/kg.d及8.2g/kg.d)有统计学差异。结论 :早产儿生后两天内应用IVIG400mg/kg 可提高血清IgG水平 ,用药组可提高用药后第7天喂养热卡数 ,停止生理性体重下降后7天内体重增长较快。Objective: To study the changing of serum IgG level and feeding tolerance after using IVIG in preterm neonates.Methods: Forty preterm neonates were randomly divided into two groups:group A IVIG 400mg/kg ,group B control cases .Administered IVIG 400mg/kg to group A within 48 hours after birth ,measured serum IgG level of each case before using IVIG and 7 days after,and then compared the two groups about the serum IgG level and feeding tolerance.Results: There were no statistics differences in the serum IgG level between the two groups before using IVIG, after using it,the serum IgG level of group A was increased .There were no differences in the gestational age,sex,birth weight,time of start feeding ,calories of start feeding ,days of physiological decline of birth weight and passing stool,there were statistics differences in the calories of feeding on the 7nd days after using IVIG and the weight gain within 7 days after physiological decline of birth weight between the two groups.Conclusion: Applied IVIG 400mg/kg within two days after birth in preterm neonates could increase serum IgG level and calories of feeding on the 7nd days,the weight gain quickly after physiological decline.
