经食道超声心动图在房间隔缺损封堵治疗中的应用

摘　　要：目的 :探讨经食道超声心动图(TEE)在引导Amplatzer封堵器堵闭房间隔缺损(ASD)术中的应用及价值。方法:应用TEE按Amplatzer封堵法适应症条件筛选6例中央型(II孔型)ASD患者,经TEE及X线引导进行封堵。结果:1例患者因合并肺动脉瓣狭窄,另1例因ASD缺损口距右房顶距离小于3mm而放弃封堵术,余4例成功进行了Amplatzer封堵术。所有病例TEE测量的ASD直径显著小于ASD最大伸展径[(18.6±1.8)mmvs(26.7±1.4)mm],术后即刻、24h、1个月行TTE检查,无残余分流及并发症。结论:TEE在ASD缺损介入治疗中占有极为重要的作用。Objective: To observe the role of TEE in guiding the Amplatzer occlusion. Methods: Six patients with ASD were treated using Amplatzer occluder under TEE and X-ray guidance. Results: Four patients were treated successfully. The mean ASD diameter measured by TEE was significantly smaller than the mean stretched diameter of the ASDs[(18.6±1.8)mm vs (26.7±1.4)mm].The cases were optimal and no residual shunt and complications was found by TTE immediately,24 hour and 1 month after procedure.One patient with Pulmonary Stenosis and another patient with gap at<3 mm to the top right atrium were given up. Conclusion: TEE is important in the transcatheter closure of ASD using Amplatzer occluder.
