摘  要:目的 :研究机械性创伤患者伤后血清可溶性白细胞介素 2受体 (sIL - 2R)的动态变化及其与创伤评分的关系 ,探讨其机械及临床意义。方法 :择机械性创伤患者 5 0例 ,按创伤评分标准分成轻度、重度、极重度三组。全部患者于伤后 2 4小时内采血 ,其中 2 5例患者进行了 12天的动态观察 ,于 3、 5、 7、 9、 12天各采血 1次。血清sIL - 2R采用ELISA法测定。结果 :创伤后 2 4小时内各组sIL - 2R明显高于正常值 (P <0 0 5 ) ,sIL - 2R的血清含量与创伤程度呈正相关 (r=0 30 6 ,P <0 0 5 )动态观察各组sIL - 2R呈波动性先升后降的变化 ,轻度患者高峰值先于重度、极重度患者出现。结论 :sIL - 2R可作为判别创伤严重程度的灵敏指标 ,它的升高说明创伤患者的机体免疫功能呈抑制状态。Objective:To investigate the postt raumatic changes of serum level of souble interleukin-2 recepor(sIL-2R)and their relationship with Injury Severity Score(ISS) and to explore the mechanism and i ts c lincal significance.Methods:Fifty patients were eligible to the study.According to the standard of ISS,the patients were divided into there groups,mild degree g roup,severe degree group and extremely severe degree group.Those of the 25 patie nts we re dynamically observed for 12 days.The blood samples of all patients were obtai ned within 24 hours,and the ones of dynamic of servations were curried out on da ys 3,5,7,9 and 12.The sIL-2R was assayed by ELISA.Results:The levels of sIL- 2R were significantly higher in patients after trauma than in normal control wit hin 24 hours( P <0 05).The dynamic observation showed that the curve change s demonstrated an increase in the early stage and then a decrease later.The clim ax of sI L-2R appeared earlier in the mild group than those in the severe and extremely s evere gro ups.The level of sIL-2R was correlated with the severity of trauma(r=0 306, P <0 05).Conclusin:The level of sIL-2R was a sensitive indes of the degree o f trauma;The increase of sIL-2R showed the immunosuppression in matic patien ts ofter trauma,there was evidence of the immunosuppressive state.
