曲张大隐静脉的病理改变与临床关系初步探讨

摘　　要：目的　探讨曲张的大隐静脉管壁病理改变与临床关系。方法　对 12 5例单纯性大隐静脉曲张和原发性深静脉瓣膜功能不全性大隐静脉曲张进行病理观察 ,同时对 2 0例同一肢体上、下三段曲张大隐静脉进行病理比较。结果　大隐静脉管壁三层结构均发生病理改变。 95 %患者表现为内膜不完整 ,中层纤维组织异常增生 ;80 %有管腔狭窄、管壁水肿、部分血栓形成 ;2 5 %患者管壁结构不完整。同一肢体上、中、下三段静脉病理改变无明显差别。结论　曲张的大隐静脉管壁存在病理性改变 ,在单纯性和原发性间无明显差异 (P >0 .0 5 ) ,在同一肢体中不同部位无明显差异 (P >0 .0 5 ) ,然于病史长短有关 ,即病史越长变化越大。Objective Study on relationshope between pathological change in great saphenous varicos and clinic.Methods 125 cases (simple varicos and primary insufficiecy function of deep venous valves)are pathologically studied to venous wall,simultanously this pathologic change is well studied to upper,middle and lower segment of this venous varicos for 20 cases.Results In 90% of all patients, internal layer of varicos is no integrant, fibrous tissue of mid - layer is anormal regeneration;in 80%,there is a number of changes of edema,stricture,thrombosis;in 25%,structure of vessel wall in no integrant. Conclusion There are pathologic changes in wall of varicos. This changes are no significant differency between simple varicos and primary insufficiency function of deep venous valaes. This changes are no significant differentiation among upper,middle and lower segment in this varicos. This changes are significant differentiation between short or and long or story,more long story more large change.
