造血干细胞移植供者TNF-α基因多态性在GVHD中的频率分析

摘　　要：探讨供者肿瘤坏死因子α(TNF α)基因多态性在重度急性移植物抗宿主病 (aGVHD)和广泛型慢性移植物抗宿主病 (cGVHD)中的作用。本研究应用变性高效液相色谱结合DNA测序检测了TNF α 30 8和TNF α 2 38基因在重度aGVHD和广泛型cGVHD的分布。结果表明 ,2 1例Ⅲ度 /Ⅳ度aGVHD病人中有 8例呈现TNF α 30 8(G/A)阳性 ,而2 8例 0度 /Ⅰ度aGVHD病人中仅 1例呈现TNF α 30 8(G/A)阳性 ,提示TNF α 30 8(G/A)在Ⅲ度 /Ⅳ度aGVHD中的频率明显高于 0度 /Ⅰ度aGVHD的病人 (P <0 .0 1)。在 2 3例广泛性cGVHD病人中有 7例呈现TNF α 30 8(G/A)阳性 ,而17例未发生cGVHD的病人中仅 1例呈现TNF α 30 8(G/A)阳性 ,提示TNF α 30 8(G/A)在广泛性cGVHD中的频率高于未发生cGVHD的病人 (P <0 .0 5 )。在 2 1例Ⅲ度 /Ⅳ度aGVHD和 2 8例 0度 /Ⅰ度aGVHD及 2 3例广泛性cGVHD及17例未发生cGVHD的病人中均有 1例呈现TNF α 2 38(G/A)阳性 ,提示TNF α 2 38(G/A)在上述各组病人间的频率无明显差异。结论 :TNF α 30 8(G/A)阳性与Ⅲ度To study the effect of donor TNF α gene polymorphism on severe acute and extensive chronic graft versus host disease (GVHD), TNF α gene polymorphism was analyzed by denaturing high perfomance liquid chromatography (DHPLC) and DNA sequencing in twenty one patients with Ⅲdegree/Ⅳdegree acute GVHD and twenty seven patients with extensive chronic GVHD. The results showed that the frequency of TNF α 308(G/A) significantly increased in patients with Ⅲdegree/Ⅳdegree acute GVHD compared to 0 degree/Ⅰdegree aGVHD patients(8/21 vs 1/28)(P< 0.01 ) and the frequency of TNF α 308(G/A) is significantly higher in patients with extensive chronic GVHD than in patients without chronic GVHD (7/23 vs 1/17)(P<0.05). However, the frequency of TNF α 238(G/A) does not significantly changed in patients with Ⅲdegree/Ⅳdegree acute GVHD and extensive chronic GVHD. In conclution, the TNF α 308(G/A) is likely to contribute to high risk for Ⅲdegree/Ⅳdegree acute GVHD and extensive chronic GVHD.
