苦寒燥湿及活血通络法治疗类风湿关节炎的临床研究

摘　　要：目的评估苦寒燥湿、活血通络法对活动期类风湿关节炎(RA)的临床疗效。方法选择100例活动期RA患者分为治疗组和对照组,治疗组予以三黄一龙汤联合注射用血塞通、非甾体抗炎药(NSAIDs)、非生物制剂抗风湿性药物(DMARDs)治疗,对照组予以NSAIDs、DMARDs治疗,两组均治疗2个疗程。观察治疗前后两组肿胀、压痛关节数,晨僵时间,评价握力及血小板、血沉、C反应蛋白等指标。结果治疗组临床和实验室指标改善早于并优于对照组(P<0.05);治疗组治疗4、8 w后,改善率分别为82%、94%,亦优于对照组的62%和80%(P<0.05)。结论苦寒燥湿、活血通络法治疗RA活动期,能更好、更快地改善临床症状、体征和实验室指标,提高治疗效果,缩短住院时间。Objective To evaluate the clinical efficiency of the treatment of traditional Chinese medicine method of bitter-cold herbs easing dampness plus Xuesetong injection promoting blood circulation to remove meridian obstruction on active rheumatoid arthritis（RA）. Methods 100 cases with active RA were divided into 2 groups：treatment group and control group,50 cases in each;NASIDs and DMARDs were administrated to the cases in both groups,while Sanhuang Yilong decoction and Xuesetong injection were added to cases in treatment group;the course lasted for 2 months;the clinical manifestation and laboratory indexes of the cases in both＆amp;nbsp;groups were observed and comparatively analyzed. Results An earlier and superior improvement in clinical manifestation and laboratory indexes was gained in treatment group than that in control group;the rates of improvement in treatment group at the time point of the 4 and 8 weeks after treatment were 82% and 94% ,respectively,much higher than those in control group（62% and 80%respectively）（P ﹤ 0. 05）. Conclusions The treatment of bitter-cold herbs easing dampness plus promoting blood circulation to remove meridian obstruction is effective on active RA;it provides a better and faster improvement in clinical manifestation,physical signs and laboratory index so that the time for hospitalization is shortened.
