双氯芬酸钠盐酸利多卡因治疗四肢急性创伤的镇痛观察

摘　　要：目的评价双氯芬酸钠盐酸利多卡因治疗四肢急性创伤性骨折及软组织损伤的镇痛疗效及安全性。方法采用随机对照试验，将符合入选标准的患者随机分为两组：试验组22例，对照组22例。试验组给予双氯芬酸钠盐酸利多卡因75mg肌注；对照组给予曲马多注射液100mg肌注。分别观察记录用药后0min、15min、30min、1h、2h、3h、4h疼痛缓解情况及不良反应。结果治疗前两组疼痛评分比较差异无统计学意义（P〉0．05），用药后0min、15min、30min、1h、2h、3h、4h试验组较对照组镇痛效果优且药物不良反应少（P〈0．05）。结论双氯芬酸钠盐酸利多卡因治疗四肢急性创伤性骨折及软组织损伤，较曲马多有更好的镇痛效果，值得临床推广。Objective To evaluate the therapeutic effect and the safety of the diclofenac sodium lidocaine hydrochloride in patients with acute traumatic fractures of the limb or soft tissue injuries. Methods A randomized double -blind control test was carried out. Twenty patients with acute traumatic fractures of the limb or soft tissue injuries were divided into two groups ： the experimental group （10 patients） which was administered 75 mg diclofanc sodium lidocaine hydrochloride as well as the control group （ 10 patients） which was administered 100 mg tramadol. The pain levels were assessed using the visual analog scales （VAS） at 0 min, 15 min, 30 min, 1 h, 2 h, 3 h and 4 h after using drugs. The incidence of side effects was also recorded. Results There was no obvious difference between two groups before treatment （ P 〉 0.05 ）. The difference of the therapeutic effect was significant between the groups at 0 h, 15 min, 30 min, 1 h, 2 h, 3 h and 4 h of using drugs （P〈0.05）. Compared to the control group, the experimental group exhibited better analgesic effect and fewer side effects. Conclusion The diclofanc sodium lidocaine hydrochloride has more significant analgesic effect and fewer adverse events than the tramadol on the treatment of patients with acute traumatic fractures of the limb or soft tissue injuries. It would be more applicable as the analgesics in the future.
