血必净对蝮蛇伤患者血清肿瘤坏死因子-α白细胞介素-1和白细胞介素-6的影响

摘　　要：目的观察血必净注射液对蝮蛇伤患者血清肿瘤坏死因子-α（TNF-α）、白细胞介素-1（IL-1）和白细胞介素-6（IL-6）的影响，探讨其治疗机制。方法418例蝮蛇伤患者随机分为常规治疗组及血必净治疗组。血必净治疗组为在常规治疗的基础上加用血必净注射液。分别于治疗前及治疗后第1、2、3、5、7天晨采静脉血，测定血清TNF-α、IL-1和IL-6水平，并进行对比。结果治疗前健康对照组及蝮蛇伤轻型、重型、危重型各组血清TNF—α、IL-1和IL-6水平两两比较差异有统计学意义（P〈0．01）。治疗前及治疗第1天常规治疗组与血必净治疗组血清TNF—α、IL-1和IL-6水平比较差异元统计学意义；第2、3、5、7天血必净治疗组血清TNF-α、IL-1和IL-6水平明显低于常规治疗组（P〈0．05）。结论应用血必净注射液对蝮蛇伤患者细胞因子的调节有积极作用。Objective To investigate the effect of Xuebijing on the level of serum tumor necrosis -α（TNF- α）, interleukins- 1 （IL- 1 ） and IL- 6 in Agkistrodo Halys snakebite patients and its protection mechanism. Methods 418 patients with Agkistrodo Halys snakebite were, according to distinct treatments, divided into normal medicine group and Xuebijing group which were treated with normal therapy in combination with Xuebijing through intravenous injection. Two groups were compared on the levels of serum TNF - α, IL - 1 and IL - 6 before therapy and during therapy. Results The increase in serum TNF - α, IL - 1 and IL - 6 level in Agkistrodo Halys snakebite patients were observed. Before treatment, serum TNF - α, IL - 1 and IL - 6 had statistical significance in health patients, mild patients, severe patients and critical ill patients （ P 〈 0.01 ）. There was no significant difference between the results within normal medicine group and Xuebijing group before and after the 1 st day of the treatment. On the 2nd, 3rd, 5th and 7th day after treatment, the levels of TNF - α, IL - 1 and IL - 6 in Xuebijing group were substantially lower than those in normal medicine group （ P 〈 0.05 ）. Conclusion Xuebijing injection has a positive impact on the inflammatory response by inhibition of TNF -α, IL - 1 and IL - 6.
