滑车上动脉对脑动脉硬化的彩色多普勒超声检测

摘　　要：目的 :通过在面部对颅内血管终末支滑车上动脉 (STCA)出颅部的彩色多普勒超声 (CDFI)检测 ,寻找一种对脑动脉硬化 (CAS)等颅内血管病变无创性检测的新方法。方法 :在 2 9例正常和 19例 CAS患者面部眶上缘内侧 STCA出颅处的眶上孔部位 ,行 CDFI及脉冲多普勒超声检测。结果 :对照组 STCA出颅部彩色血流信号及多普勒频谱检出率为 10 0 %,其位置固定 ,易于探测 ,频谱特征与眼动脉相似 ,为正向三峰型 :SP1 、 SP2 、 DP峰 ,SP1 >SP2 。 CAS患者 SP2 升高 ,接近或超过 SP1 ,DP高于对照组 (P<0 .0 5～ 0 .0 1) ,PI、 RI低于对照组 (P<0 .0 0 1)。结论 :STCA的 CDFI检测是一种准确、方便、价廉、无损伤性、易于掌握、便于推广的诊断 CAS等脑血管病变的新方法。Objective:To explore a new and non-injured method for cerebral arteriosclerosis(CAS)through detecting supratrochlear artery(STCA)which was the end branch of intracranial vascular at face.Methods:Detecting STCA of 29 normal controls and 19 CAS at facial supratrochlear artery of supraorbital foramen by color and pulsed Doppler ultrasound.Results:100 Percentage STCA could be displayed at supraorbital foramen because it is at the superficialis and same position,so easy to detect.There were similar frequency spectrum between STCA and ophthalmic artery with three up wares:SP 1,SP 2,DP and SP 1>SP 2 of systole in control group.The Sp 2 of frequency spectrum and DP(Vd)of protodiastole in CAS were higher than that in control group(P<0.05～0.01)while PI,RI were lower(P<0.001).Conclusions:Color Doppler ultrasound is an accurate,non-injured,convenient and cheap new method in diagnosing CAS through detecting STCA.It is easy to master and spread.
