吻合血管尺动脉腕背支为蒂双叶皮瓣的临床应用

摘　　要：目的 :探讨尺动脉腕背支为蒂开支与降支分别为营养血管的双叶游离皮瓣在手指外伤中的应用。方法 :9只成人新鲜上肢标本 ,左 4只 ,右 5只。于尺动脉腕背支起始处灌注红色乳胶 ,手术显微镜下解剖观察尺动脉腕支及其升支与降支起始、行程、分支及吻合情况。临床设计以尺动脉腕背支为蒂 ,腕背支的升支与降支分别为营养血管的双叶游离皮瓣修复单指皮肤全脱套伤 8例 ,示中指及中环指背或指掌侧皮肤缺损 7例。结果 :临床应用 15例 ,皮瓣全部成活 ,术后随访 6～ 15个月 ,伤指外形及皮肤色质与健指相似 ,掌侧远端指腹两点辨觉为 3.2～ 6 .1mm ,运动功能按TAM系统标准评价 :优 6指 ,良 8指 ,可 4指。结论 :尺动脉腕背支为蒂升支与降支分别为营养血管的双叶游离皮瓣 ,是修复单指皮肤全脱套伤及相邻两指背或掌侧皮肤的一种良好方法。Objective: To introduce the double skin free flaps pedicled with the dorsal carpal branch of ulnar artery in the digital wound.Methods:9 fresh cadaveric upper extremities were used.Red dye was injected in the dorsal carpal branch of ulnar artery.The origin,course,branches,distribution and anastomosis were observed under microscope.On the basis,design the double skin free flaps pedicled with the dorsal carpal branch of ulnar artery to repair the injuries of fingers.Results:This method was performed in 15 cases.All the transplants survived.Fellow-up for 6～15months after operation was done.The appearance and function of finger were satisfactory.Conclusions:This procedure provides a good method for repairing the skin tissue defects of the volar or dorsal of a finger and the injuries of a finger.
