复发性结节性非化脓性脂膜炎7例临床分析

摘　　要：目的　通过回顾性分析 ,进一步探讨、了解复发性结节性非化脓性脂膜炎的病因、临床表现、治疗和预后。方法　回顾分析 196 2— 2 0 0 0年经病理确诊、随访的 7例复发性结节性非化脓性脂膜炎。结果　 7例患者均有发热、皮下结节。肺部口罗音 5例 ,腹痛 4例 ,淋巴结肿大 3例 ,肌肉关节痛 2例 ,多浆膜腔积液 2例 ,肝大 2例 ,脾大 1例。病因分析中 ,3例合并感染 ,1例外伤 ,2例结核。 1例合并肝性脂膜炎。随访中 ,3例死亡 ,3例痊愈 ,3例好转 ,1例失访。结论　复发性结节性非化脓性脂膜炎临床表现多样 ,诊断较为困难 ;一旦确诊后 ,应注意相应病因诊断 ;治疗上除应用糖皮质激素或免疫抑制剂 。Objective The purpose of this study was to investigate the etiology,clinic featuer,therapy and prognosis of recurrent nodular nonsuppurative (Weber Christian disease).Methods The clinical files of 7 histologic proven panniculitis cases at the First Hospital Affiliated to Chongqing University of Medical Sciences from 1962 to 2000 were reviewed retrospectively.Results Tender subcutaneous nodules and relapsing febrile episodes or acute onset of high fever occurred in all 7 cases.Other systemic manifestations due to visceral involvement were present,which include rales,abdominal pain,swelling of lymph nodes,myalgia and arthralgia and hepatomegaly,etc.Three cases were associated with infection,2 associated with tuberculosis and 1 with trauma.Three cases recovered from panniculitis,3 died of panniculitis with complications.One lost during follow up.Conclusion The different types of panniculitis are associated with a wide range of etiologies and systemic manifestations.Treatment depends upon eliminating the underlying cause in addition to using anti inflammatory and immunosuppressive agents.
