摘 要:目的 为了解脱水对糖尿病造影剂肾病发生的影响 。 方法 建立单侧肾切除糖尿病大鼠模型 ,比较脱水与否对糖尿病造影剂肾病发生的影响 ,及血浆内皮素 -1(ET-1)变化 。结果 在脱水情况下 ,造影剂可造成糖尿病模型鼠急性肾功能减退 ,而在不脱水情况下 ,肾功能减退不明显。在两种情况下 ,注射造影剂后 ,血浆ET-1浓度变化均不明显。结论 糖尿病 ,特别是伴肾功能不全时 ,如果存在脱水 ,则造影剂肾病 (CMN)发生率明显增加 ,而与循环中ET-1无关。Objective To investigate the effect of dehydration on radiographic contrast nephropathy ( RCN ) in diabetic rats. Methods The rats were randomly divided into following groups: normal control rats, diabetic ( streptotocin-induced ) and uninephrectomized ( Unx ) rats, diabetic and Unx rats with free water plus diatrizoate injection (10ml/kg), diabetic and Unx rats without free water but diatrizoate injection (10ml/kg). Renal function was measured before and after injection, and plasma endothelin-l (ET-1) was also measured. Results Diatrizoate reduced the glomerular filtration rate (GFR) in the diabetic and Unx rats with dehydration, whereas it didn't significantly decrease the GFR in the diabetic and Unx rats with free water. The plasma EF-1 levels in diabetic and Unx rats were lower than that in control rats, diatrizaote had little effect on the plasma ET-1 concentration. Conclusion Dehydration has a significant effect on renal response to contrast media in diabetic and Unx rats. The decrease of renal response to contrast media may have nothing to do with the plasma ET-1 level.
