高压氧治疗新生儿缺氧缺血性脑病的远期疗效

摘　　要：目的　探讨高压氧治疗新生儿缺氧缺血性脑病 (HIE)的远期疗效。　方法　治疗后 4～ 5年对高压氧 (HBO)治疗组 32例 ,对照组 (同期未用HBO治疗者 ) 2 8例进行随访并做详细的体格及神经系统检查 ,观察两组的远期疗效。　结果　治疗组丹佛氏智测异常率低于对照组 (P =0 .0 0 0 3) ,HIE预后不良率亦较对照组降低 (中度 :P =0 .0 2 ;重度 :P =0 .0 1) ,视听检查均无异常发现。　结论　HBO治疗HIE能改善HIE患儿智力发育并降低预后不良率 ,且无减压病及视听异常等副作用发生。Objective To explore the long term follow up results of hyperbaric oxygen therapy(HBO) on neonates with hypoxic ischemic encephalopathy(HIE). Methods After 4～5 years of discharged from our hospital,thirty two cases of HIE neonates with HBO therapy (treatment group) and other 26 cases of HIE neonates without HBO therapy(control group) were followed up and assessed with their physical and neurological examinations,hearing and visual acuity detections. Results The abnormal rates of Denver development screening test(DDST),and the frequencies of unfavorable prognosisof treatment group were lower than those of control group(P=0.0003;and Ⅱ:P=0.02;Ⅲ:P=0.01). No abnormalities in hearing and retinal detection were showed. Conclusinon HBO may improve the mental status and decrease the unfavorable prognosis of HIE neonates.There are no side effects such as decompression disease,abnormalities of visual and hearing functions after HBO.
