摘 要:目的 :对比研究两种剂量氟碳 (PFC)部分液体通气 (PLV)对蒸气吸入性损伤犬的血气和血流动力学的影响。方法 :将健康犬 15条随机分成两组 ,即PFC 6ml/kg组 (PFC 6 )和PFC 12ml/kg(PFC 12 )组。采用蒸气吸入造成吸入性损伤模型后经气管内注入PFC实施部分液体通气 (PLV)治疗 ,观察治疗后 30min、6 0min、90min血气、血流动力学参数的变化。结果 :PFC 6组PaO2 在PLV治疗 30min后明显升高 (P <0 .0 5 ) ,在治疗 6 0min、90min后的PaO2 与致伤值比较无统计学意义 ,而PFC 12组的PaO2 在PLV治疗后 30min、6 0min、90min均明显升高 (P <0 .0 5 )。与PFC 6组相比 ,PFC 12组的PaO2 、PvO2 均呈上升趋势 ,但未达显著水平 (P >0 .0 5 ) ;在PLV治疗后各时间点的PaCO2 与HR、MAP、CVP与致伤值相比均无明显变化 (P >0 .0 5 ) ,两组之间比较 ,各对应时间点的PaCO2 、HR、MAP、CVP也均无明显变化 (P >0 .0 5 )。结论 :氟碳部分液体通气是一种新的通气技术 ,它能明显改善蒸气吸入伤犬的氧合作用 ,对血流动力学参数无不良影响 ;两种剂量氟碳部分液体通气治疗蒸气吸入性损伤未呈现明显剂量 -效应关系。Objective:To compare effects of two different doses of perfluorocarbon with partial liquid ventilation on arterial and mixed venous blood gas and hemodynamics in treatment of dogs with steam inhalation injury. Methods:The fifteen dogs with steam inhalation injury were randomly divided into two groups.Group 1 was treated with PLV of 6ml/kg of perfluorocarbon that was instilled slowly into the lung. Group 2 was treated with PLV of 12 ml/kg.Results:PaO 2 in group 1 improved significantly at 30 min after PLV treatment but did not significantly at 60 min and 90 min after PLV treatment. However,PaO 2 in group 2 improved significantly at all 30 min, 60 min, and 90 min after PLV treatment ( P <0.05).Although PaO 2 and PvO 2 in group 2 improved, they had no statistics significance ( P >0.05)compared with those in group 1. PaCO 2,HR,MAP,and CVP in two groups did not have obvious changes at any time point after PLV treatment( P >0.05).Conclusion: Partial liquid ventilation with perfluorocarbon is a new ventilation technology which improves significantly PaO 2 and decreases Qs/Qt.There is no impairment of hemodynamics in PLV.Neither doses of perfluorocarbon PLV shows a dose dependent manner in the treatment of inhalation injury.
