摘 要:目的 探讨新生儿持续性肺动脉高压 (PPHN)新的治疗方法。方法 采用实验小猪制成急性肺动脉高压的动物模型来模拟PPHN ,从静脉输入不同剂量的三磷酸腺苷 (ATP) ,观察对肺动脉高压的疗效。结果 静脉滴注ATP 2 5~ 5 0 μg/(kg·min)后肺动脉压力 (PAP)显著下降 ,肺动脉收缩压 (PASP)、肺动脉舒张压 (PADP)和肺动脉平均压 (PAMP)分别从 5 7± 1 1kPa、3 9± 0 7kPa和 4 8± 0 8kPa下降至 4 0± 0 9kPa(P <0 0 5 )、2 8± 0 7kPa(P <0 0 5 )和 3 5± 0 8kPa(P <0 0 5 ) ;体循环血压 (SAP)无明显变化 ,心率 (HR)、心输出量 (CO)和心指数 (CI)轻度增加 ,血氧分压 (PO2 )和血氧饱和度 (SaO2 )也有改善 ,但经统计学分析无显著差别 (P >0 0 5 )。当ATP剂量≥ 30 0 μg/ (kg·min)时 ,SAP显著下降 ,HR减慢 ,PAP反而明显升高。 结论 ATP治疗肺动脉高压有显著疗效 ,可迅速降低肺动脉高压 。Objective To explore a new method of the treatment for persistent pulmonary hypertension of newborn(PPHN) Methods Acute pulmonary hypertension models of experiment piglets were prepared to mimic PPNH lntravenous adenosine triphosphate (ATP)was administrated with different doses to observe the effects on pulmonary hypertension Results There was significant decrease of pulmonary arterial pressure (PAP)after infusion of ATP 25~50 μg/(kg·min).The pulmonary arterial systolic pressure(PASP),pulmonary arterial diastolic pressure(PADP)and pulmonary arterial mean pressure(PAMP)were decreased from 5 7±1 1 kPa,3 9±0 7 kPa and 4 8±0 8 kPa to 4 0±0 9 kPa(P<0 05)?2 8±0 7 kPa(P<0 05)and 3 5±0 8 kPa(P<0 05) respectively Systemic arterial blood pressure(SAP)did not significantly change Heart rate (HR),cardiac output(CO)and cardiac index (CI)were slightly increased and PO 2 and SaO 2 were also improved,but there were no significant differences (P>0 05) When ATP dose was ≥300 μg/(kg·min),we found that the SAP and HR decreased significantly, on the contrary, the PAP significantly increased. Conclusion ATP has good effects in the treatment of pulmonary hypertension. It can decrease pulmonary hypertension quickly,but the dose of ATP must be appropriate
