血清、胸腔积液结核抗体联合检测对结核性胸膜炎的诊断价值

摘　　要：目的 :探讨血清、胸腔积液中结核分支杆菌特异性抗体 (PPD IgG)联合检测对结核性胸膜炎 (TP)的诊断价值。方法 :用ELISA法 ,对 32例结核性胸膜炎、16例肺癌合并胸腔积液、9例漏出性胸腔积液进行血清及胸腔积液PPD IgG联合检测 ,同时做卡介菌 纯蛋白衍化物 (BCG PPD)皮试 ,以及胸腔积液浓缩厚涂片找结核菌和胸腔积液结核菌培养。结果 :TP组血清PPD IgG检测 2 8例 (87.5 0 % )阳性 ,胸腔积液PPD IgG检测 30例 (93.75 % )阳性 ;肺癌组血清PPD IgG检测 3例 (18.75 % )阳性 ,胸腔积液PPD IgG检测 1例 (6 .2 5 % )阳性 ;漏出液组血清PPD IgG检测 1例 (11.11% )阳性 ,胸腔积液PPD IgG检测阳性率为 0。三组间差异均有显著性 (P <0 .0 0 5 )。结论 :血清、胸腔积液PPDObjective:To study the diagnostic value of combinative test of mycobacterium tuberculosis special antibody(PPD IgG) in serum and pleural effusion for tuberculous pleuritis(TP).Methods:PPD(purified protein derivative) IgG in serum and pleural effusion was examined by ELISA method in 32 patients with TP, 16 patients with lung cancer combined with pleural effusion and 9 patients with leaky pleural effusion.PPD skin test,condensed thick smears for tubercle bacillus seeking in pleural effusion and tubercle bacillus culture were utilized in all patients at the same time.Results:In TP group,serum PPD IgG postive were tested in 28 cases(87.50%) and pleural effusion PPD IgG positive in 30 cases(93.75%).In lung cancer group,serum and pleural effusion PPD IgG positive were only tested in 3 cases(18.75%) and 1 case(6.25%),respectively.But in leaky pleural effusion group,only 1 case of PPD IgG positive was tested in serum samples,and PPD IgG postive were not found in pleural effusion samples.Significant statistics difference was shown in the three groups( P < 0.005 ).Conclusions:Our results suggest that combination test of serum and pleural effusion PPD IgG plays an important role in diagnosis of tuberculosis pleurisy.
