摘  要:目的 探讨低温体外循环病人蛋白C系统变化及其意义。方法 选择 2 0例低温体外循环病人 ,于转流前、转流中、转流结束后即刻 ,转流后 5h ,术后第一天分别测定血浆血栓调节蛋白、游离蛋白S、D 二聚体抗原水平及蛋白C活性。结果 转流中 ,转流结束后即刻 ,转流后 5h ,术后第一天血浆血栓调节蛋白、D 二聚体水平较转流前显著增高 ;血浆游离蛋白S水平及蛋白C活性较转流前显著降低。结论 蛋白C系统紊乱可能与低温体外循环病人纤溶活性增强及出血倾向有关。Objective To realize siginificance of the changes of protein C system in patients undergoing hypothermic cardiopulmonary bypass.Methods The plasma antigen levels of thrombomodulin,free protein S and D dimer and protein C activity were determined before cardiopulmonary bypass(CPB),during and immediately after CPB,5h after CPB and on the first day after operation.Results During and immediately after CPB,5h after CPB and on the first day after operation,the plasma level of thrombomodulin and D dimer were significantly increased, free protein S plasma level and protein C activity decreased compared with that before CPB.Conclusion These results suggest a disturbance of the protein C system ,which is possibly linked to increase of fibrinolytic activity and the bleeding tendency in patients undergoing hypothermic cardiopulmonary bypass.
