抗原捕获试验在诊断特发性血小板减少性紫癜中的应用价值

摘　　要：目的 :探讨抗原捕获试验在诊断特发性血小板减少性紫癜 (ITP)中的应用价值。方法 :用抗原捕获试验检测 4 2例 ITP患者和 2 1例继发性血小板减少性紫癜 (STP)患者的抗血小板糖蛋白 b/ a抗体 (抗 GP b/ a Ab) ,酶联免疫竞争抑制试验检测血小板相关抗体 (PAIg) ,比较二者对 ITP诊断的敏感性和特异性。结果 :抗原捕获试验检测 ITP患者抗 GP b/ a Ab敏感性为 73.8% ,特异性为 90 .5 % ;酶联免疫竞争抑制试验检测 ITP患者 PAIg敏感性为 71.4 % ,特异性为 2 3.8%。结论 :抗原捕获试验检测抗 GP b/ a Ab在Objective:To evaluate the value of the antigen capture assay in the diagnosis of idiopathic thrombocytopenic purpura(ITP).Methods:Forty-two patients with ITP and twenty-one patients with secondary thrombocytopenic purpura(STP) were studied.Their autoantibodies against platelet glycoprotein antigens Ⅱb/Ⅲa(Anti GPⅡb/ⅢaAb)were assessed by the antigen capture assay and their platelet-assiciated Ig(PAIg) were measured by competitive enzyme-linked immunoassay.Compare their sensitivity and specificity in the diagnosis of ITP.Results:We reported a sensitivity of 74% and specificity of 90% for antigen capture assay versus a sensitivity of 71% and specificity of 24% for competitive enzyme-linked immunoassay in patients with ITP.Conclusions:Our results suggested that antiGPⅡb/ⅢaAb assessed by antigen capture assay was useful in the laboratory diagnosis of ITP.
