摘 要:目的 探讨单纯性肥胖儿童和非肥胖儿童脂肪细胞的基础凋亡状况及其相关因素。方法 应用DNA原位末端标记技术 (terminaldeoxynucleotidyltransferase mediateddUTPnickendlabeling,TUNEL)检测 15例单纯性肥胖儿童 (男 11例 ,女 4例 )和 15例非肥胖儿童 (男 9例 ,女 6例 )腹壁皮下脂肪细胞的基础凋亡状况 ,计算凋亡指数 (apoptosisindex,AI)并进行对比和相关性分析。结果 单纯性肥胖儿童和非肥胖儿童腹壁皮下脂肪细胞存在很低的凋亡率 ,肥胖组AI为 (0 2 3± 0 18) % ,非肥胖组为 (0 6 5± 0 5 9) %。单纯性肥胖儿童脂肪细胞的凋亡率明显低于非肥胖儿童 (P <0 0 2 ) ;儿童腹壁皮下脂肪细胞的AI与体块指数 (bodymassindex ,BMI)、体脂百分数 (percentbodyfat,BF % )和体脂含量(fatmass,FM)呈负相关 ,与性别和年龄则无明显相关性。结论 儿童腹壁皮下脂肪细胞的AI与BMI和FM呈负相关 ,单纯肥胖儿童脂肪细胞的凋亡率明显低于非肥胖儿童 。Objective It has been demonstrated in recent years that adipocytes can undergo apoptosis in vitro and in vivo, but most studies were conducted with 3T3 L1 murine cell lines and animal models. The pattern of adipocytes apoptosis in human and its relationship with obesity remain unknown. The present study was designed to explore the basal apoptosis of adipocytes and its related factors in obese and non obese children. Methods Abdominal subcutaneous adipose tissue samples were obtained from thirty patients, including fifteen obese children (male/female: 11/4) and fifteen non obese children (male/female: 9/6) who underwent appendectomy for acute appendicitis. The tissues were fixed with 10% neutral formaldehyde solution and 7 μm thick sections were cut from paraffin embedded fat tissues. Adipocytes apoptosis was detected with In Situ Cell Death Detection Kit(terminal deoxynucleotidyl transferase mediated dUTP nick endlabeling, TUNEL), and the percentage of apoptosis positive nuclei (apoptosis index, AI) was calculated. Results AI of adipocytes in obese and non obese children was very low. The AI of adipocytes in obese group was (0.23±0.18)%, and the AI of adipocytes in non obese group was (0.65±0.59)%. The AI of adipocytes in obese children was significantly lower than that in non obese children ( P <0.02). At the same time, the AI of adipocytes in all the studied children had a negative correlation with BMI, BF% and FM, but had no correlation with age and gender. Conclusion The AI of adipocytes in children had a negative correlation with body fatness. The apoptosis rate of adipocytes in obese children was siginificantly lower than in non obese children, this may be involved in the onset of obesity.
