摘  要:目的 :评估原发性高血压病人内皮素 (ET)、一氧化氮 (NO)、ET/NO值的变化以及ET、NO二者比例的变化与颈动脉重构间的相互关系。方法 :选取原发性高血压病人 43例 (原发性高血压组 ) ,健康自愿者 40例 (正常对照组 )作为观察对象。应用二维超声测定颈动脉结构与功能改变 ,应用放射免疫法测定ET ,应用Griess法测定NO含量。  结果 :原发性高血压组NO较正常对照组显著降低 ,而ET、ET/NO值均较正常对照组显著升高 ,有显著性差异 (P均 <0 .0 5 )。NO与原发性高血压组颈动脉内膜—中层厚度 (IMT)呈负相关 (γ =-0 .2 6 7,P <0 .0 5 ) ,与颈动脉扩张性呈正相关 (γ =0 2 97,P <0 .0 1)。ET、ET/NO值与原发性高血压组颈动脉IMT呈正相关 (γ =0 .2 98、γ =0 .2 0 3,P均 <0 .0 1) ,与颈动脉扩张性呈负相关 (γ =-0 .30 3、γ =-0 .172 ,P均 <0 .0 5 )。  结论 :原发性高血压病人ET水平较健康人增加 ,而NO较健康人降低 ,二者存在比例失衡。ET。Objective:The purposes of this study were to investigate changes of endothelin(ET),nitric oxide(NO)and the ratio of ET to NO,and to evaluate relationship between carotid remodeling and ET,NO and ET/NO ratio in hypertensive patients. Methods:Forty-three essential hypertensive patients (EH group) and forty normotensive controls (NC group) were observed.Carotid remodeling was determined by high-resolution B-mode ultrasonography,ET by radioimmunoassay and NO by Griess′method. Results:NO was significantly lower in EH group than in NC group (p<0.05).ET and ET/NO were significantly higher in EH group than in NC group (p<0.05).NO was negatively correlated with intima-media thickness (IMT),and was positively correlated with carotid disentsibility (CD) in EH group.ET and ET/NO were positively correlated with IMT,and were negatively correlated with CD in EH group. Conclusions:In hypertensive patients.ET level is increased,but NO level is decreased.The ratio of ET to NO is imbalanced.Carotid remodeling is closely related with ET,NO and imbalanced ET to NO ratio in hypertensive patients.
