摘  要:目的 探讨氧疗时新生儿血浆中抗氧化水平的动态变化。方法 选择需氧疗的新生儿 3 8例 ,其中早产儿 2 0例 (A组 ) ,足月儿 18例 (B组 )。分别于吸氧前、吸氧后 3d、7d各测血浆中还原型谷胱甘肽 (GSH)、谷胱甘肽过氧化酶 (GSH px)、谷胱甘肽S转移酶 (GST)、丙二醛 (MDA)含量的变化。并以同期住院的不需氧疗的早产儿 2 0例 (C组 )及正常足月儿 (D组 )作为对照组。结果 ①GSH :早产儿出生时GSH水平较足月儿低 (P <0 0 5 ) ,A组吸氧 3d及 7d时GSH较给氧前 (0d)时明显下降 ,且与对照组相比 ,差异具有显著性意义 (P <0 0 5 )。②GSH px :早产儿出生时GSH px水平早产儿较足月儿低 (P <0 0 5 ) ,B组GSH px活性于吸氧 3d[(76 0 0±6 3 9)U/L]及 7d[(87 3 5± 7 80 )U/L]时较 0d[(66 65± 7 65 )U/L]明显增强 ,且 7d时更为显著 (P <0 0 1) ;而A组变化却不明显。③GST :早产儿出生时GST含量较足月儿高 (P <0 0 5 ) ,而氧疗后 3d均出现明显下降。④MDA :氧疗后 7d时MDA含量A组 [(6 80± 0 92 ) μmol/L]、B组 [(4 2 7± 0 76) μmol/L]分别较 0d[(5 48± 1 0 3 )μmol/L、(2 98± 0 5 6) μmol/L]明显增加 (P <0 0 5 ) ,与对照组比较差异具有显著性意义 (P <0 0 5 )。结论 血浆中GSH、GSH px。Objective To detect antioxidant levels in newborns with oxygen therapy.Methods Thirty eight newborns with oxygen therapy,including twenty preterm newborns(Group A) and eighteen full term newborns (Group B), were enrolled into the study.Glutathione (GSH), Glutathione S transferase (GST), Glutathione peroxidase (GSH ps) and malondialdehyde (MDA) concentrations were measured before, 3 days and 5 days after the oxygen therapy. Twenty preterm newborns without oxygen therapy (Group C) and eighteen full term healthy newborns (Group D) were served as controls.Results ①GSH:GSH level in preterm newborns at birth was reduced significantly than that in full terms. GSH level in Group A was reduced at the 3 and 5 day after the oxygen therapy (P<0 05).②GSH px:GSH px level in preterm newborns at birth was reduced significantly compared to the full terms (P<0 05). GSH px activity in Group B was increased significantly at the 3 day (76 00±6 93U/L) and 5 day (87 35±7 80U/L) following the oxygen therapy, whereas GSH px activity in Group A changed little in response to the same regimy. ③GST: GST content, which was higher in preterm newborns at birth than that in full terms(P<0 05), was decreased significantly 3 days after the therapy. ④MDA: MDA concentrations in both Group A and B were increased significantly 7 days following the oxygen supplement, compared with those in control group (P<0 05).Conclusions The change in GSH, GSH px, GST and MDA might be of value to the assessment of oxidative stress in newborns.
