摘 要:目的用大鼠心脏移植模型观察左旋精氨酸(L-Arginine,L-Arg)对大鼠移植心的保护作用,并探讨其作用机制。方法采用大鼠异位心脏移植模型,将Wistar大鼠随机分为4组:空白对照组,单独给以L-Arg组,环孢素(cyclosporine,CsA)组,CsA加L-Arg组。观察移植心存活时间,在移植心停跳后取血查硝酸盐/亚硝酸盐含量,并将移植心做病理学检查。结果各组的存活时间分别为:对照组(11.1±1.2)天,L-Arg组(14.4±4.1)天,CsA组(57.0±18.8)天,CsA加L-Arg组(115.9±43.3)天。给予L-Arg组血硝酸盐/亚硝酸盐含量对照组(121.14±15.78)μmol/L和CsA加L-Arg组(132.85±14.40)μmol/L比给予普通饲料组的对照(83.86±14.71)μmol/L和CsA组(83.49±14.53)μmol/L高,差异有显著意义(P<0.05)。形态学损伤对照组和L-Arg组间无明显差异,但是长期存活的大鼠移植心冠状血管改变却得到改善。结论L-Arg对大鼠心脏移植后急性排斥保护作用较弱,对长期存活移植心的进行性动脉硬化有一定的保护作用,其对血管内皮细胞的保护作用及抑制血管平滑肌细胞增生可能参与了这一保护机制。Objective The aim of this study was to assess the efficacy of L-Arginine(L-Arg)along and in combination with cyclosporine(CsA)on allograft survival in rat cardiac transplant model.Methods Animals were fed with either standard diet or diet including5%L-Arg.Four groups were studied;these consisted of untreated recipients fed either standard diet or diet including L-Arg;recipients treated with CsA and fed with either standard diet or diet included L-Arg.The survival time of allograft were observed,the concentration of nitrate/nitrite in rat serum were tested too.Histological findings were observed by light microscope.Results Untreated animals fed standard diet(group1,n=7)rejected their allografts at11.1±1.2days,while those fed diet included L-Arg(group2,n=9)had graft survival of14.4±4.1days(P>0.05versus group1,P<0.05vs group3,4).Animals treated with CsA alone and standard diet(group3,n=8)rejected at57.0±18.8days(P<0.05vs Groups1,2,4)while the combination of CsA and diet included L-Arg(group4,n=10)rejected at115.9±43.4days(P<0.05vs Groups1,2,3).The concentration of nitrate/nitrite of those animals fed with diet including L-Arg(group2:121.14±15.78μmol/L;group4:132.85±14.40μmol/L)were higher than that of those animals fed with standard diet(group1∶83.86±14.71μmol/L;group3∶83.49±14.53μmol/L)(P<0.05).The histological findings showed the injury in group1was the same as that in group2,but group4was less than group3.Conclusions L-Arg had protective effect on cardic allograft in rats,but the effect is not so prominent in acute rejection.L-Arg protected the vasa coronaria of allograft from accelerated atherosclerosis,the mechanism was not clear.
