胎盘分级BPD及羊水超声显像对预测胎儿成熟度价值的研究

摘  要:通过377例妊娠晚期的胎盘分级、胎头双顶径(BPD)及羊水超声显像观测,对其预测胎儿成熟度的价值进行了研究。结果:(1)胎盘Ⅲ级随着孕龄的增加而持续增多,过期妊娠时达16%;(2)BPD≥8.6cm者98.6%、BPD≥8.7cm100%及羊水过少100%发生于37周以上妊娠;(3)Ⅲ级胎盘和BPD≥8.7cm时,L/s均≥2,新生儿体重皆≥2500g,二者无RDs发生。羊水过少常伴有胎儿过熟(75%);(4)三者具有相关关系,可以相互印证;(5)B型超声预测胎儿成熟度的可信性与生化测定是相同的。作者认为利用B型超声预测胎儿成熟度以妊娠37周以上价值大,胎盘Ⅲ级、BPD≥8.7cm应视为胎儿发育成熟的超声指标,羊水过少的超声诊断应成为临床终止妊娠的指征。B型超声显像可作为替代羊水穿刺预测胎儿成熟度的良好方法。The ultrasonography of placental grading, biparietal diameter (BPD) and amniotic fluid in 377 cases at late period of pregnancy are studied and evaluated. Results as follows: (1) The number of grade Ⅲ placenta persistently increases with the age of pregnancy, and amounts to 35% in post-term pregnancy. (2) BPD ≥8.6cm of 98.6%, BPD≥8.7cm of 100% and oligohydromnios of 100% occurs at over 37th gestation week. (3) In grade Ⅲ placenta and BPD ≥8.7cm group, all L/S are more than or equal to 2, the weight of neonatuses all more than or equal to 2500g, and no respiratory distress syndrome (RDS) occured oligohydromnios is often accompanied with fetal post-maturation (75%). (4) There is a correlationship between placental greding, BPD and amniotic fluid which can confirm each other for predicting fetal maturity. (5) Accuracy of predicting fetal maturity by B-mode ultrasound is identical with biochemistric examination. The authors believe that using B-mode ultrasonography to predict fetal maturity is of great value in pregnancies of more than 37 weeks. It is suggested that grade Ⅲ placenta and BPD≥8.7cm may be the ultrasonic indices for predicting fetal maturity and oligohydromnios may serve as a index for the ending of pregnancy.
