摘　　要：目的 :探讨三维彩色多普勒超声在产科的临床应用价值。方法 :应用 Voluson 530 DMT成像系统对 34例中晚期妊娠孕妇作二维及三维彩色多普勒超声检查。由两位经验丰富的检查者分别判断三维超声显示血管的走行、分布等方面能否提供更丰富的诊断信息。结果 :1 .三维彩色多普勒超声可清楚显示脐带 -胎盘循环、脐带、脐带 -胎儿循环及胎儿体内循环等的立体形态及空间关系 ;2 .在 34例孕妇中 ,两位检查者均认为其中 30例三维超声可提供更丰富血流信息 (88.2 % ) ,二者判断符合率为 97.1 %。 Kappa=0 .83,说明二者在判断三维图像与二维图像的结果方面有相当满意的一致程度。结论Objective:To evaluate the clinical value of three dimensional color Doppler ultrasound in obstetrics.Methods:Thirty four pregnant women in second and third trimester were examined by two dimensional and three dimensional color Doppler ultrasound.Two skillful inspectors evaluated the results that whether three dimensional ultrasound could provide more diagnostic information in displaying the course and distribution of the vessels.Results:1 Three dimensional color Doppler ultrasound could demonstrate the whole shape and positional relation of umbilical cords,cord placenta circulation,cord fetus circulation,and internal circulation of the fetus.2.In 30 of 34(88 2%)cases,both of the inspectors considered that three dimensional ultrasound could provide more flow information,and the coincident rate was 97 1%.The Kappa value was 0 83,which confirmed the judgement consistency of two inspectors.Conclusions:Three dimesnional color Doppler ultrasound has important clinical value and prospect in obstetrics.
