摘 要:用心导管方法观察19例肺心病和10例慢阻肺患者运动前后肺循环动力学参数的变化。发现静息时肺心病组肺动脉压、肺血管阻力均高于慢阻肺组(P<0.01),肺心病组、慢阻肺组分别有12例(占63%)和2例(占20%)存在肺动脉高压(PAH)。运动后心输出量和肺血管阻力增加,两组肺动脉压均明显升高(P<0.01),肺心病组15例出现PAH(占79%),慢阻肺组无一例出现。本文还对肺心病患者无PAH现象作了初步探索。19 cases with cor pulmonale (Group Ⅰ) and 10 patients with chronic obstructive pulmonary disease (COPD, Group Ⅱ) were examined with Swan-Canz catheter. It was found that the pulmonary artery pressure and the pulmonary vascular resistance in the Group Ⅰ are higher than that in the Group Ⅱ at rest ( P<0.01 ) and that the pulmonary artery hypertension ( PAH) existed in 12 cases of Group Ⅰ ( 63%) and 2 cases of Group Ⅱ (20%). after exercise, 15 cases showed PAH in Group Ⅰ (79%), but none in Group Ⅱ. The increase of the cardiac output and the pulmonary vascular resistance was responsible for the marked elevation of the pulmonary artery pressure in both groups ( P<0.01 ) . The reason of cor pulmonale without PAH was primarily discussed.
