下肢深静脉血栓的经动脉溶栓治疗

摘　　要：目的 :探讨经动脉保留导管溶栓治疗下肢深静脉血栓的应用价值。方法 :40例下肢深静脉血栓患者经健侧股动脉穿刺插管至患侧髂股动脉内 ,并保留导管 3～ 7d ,经导管注入肝素 3 0 0 0～ 5 0 0 0IU及尿激酶 2 5万～ 10 0万IU稀释液5 0～ 2 0 0ml,每日 1次。另 40例经足背静脉注入同样的药物。共植入下腔静脉滤器 3 1例 ,髂股静脉支架植入术 10例。结果 :80例患肢肿胀、疼痛均于溶栓治疗后 1～ 3d内开始消退、减轻。 3 1例滤器均未发生移位变形 ,下腔静脉通畅 ,10例支架无阻塞。结论 :经动脉保留导管溶栓治疗全下肢深静脉血栓形成 ,安全有效。Objective:To evaluate intraarterial thrombolysis in the treatment of lower extremity deep venous thrombosis (LEDVT).Methods:40 patients of LEDVT were treated with transarterial thrombolysis, 3000～5000IU of heparin and 0.25～1 million IU of urokinase were infused once a day via ipsilateral iliofemoral artery for 3～7 days. In the other 40 cases, the same drugs were infused through the pedal vein.31 inferior vena cava (IVC) filters and 10 stents in iliaca femoral vein were implanted. Results:The pain and swelling of lower limbs of 80 patients got relief within 1～3 days after thrombolysis. There were no filter migration and deformation. IVC remained patent.Conclusion:Transarterial thrombolysis is a safe and effective method in the treatment of lower extremity deep venous thrombosis.
