摘　　要：目的 :研究茵陈承气汤对大鼠急性出血坏死性胰腺炎腺泡细胞凋亡及调控基因Bax表达水平的影响。 方法 :胰胆管内注射去氧胆酸钠制备大鼠急性出血坏死性胰腺炎模型 ,随机分为对照组 ,模型组 ,中药 (茵陈承气汤 )治疗组 ,观察各组大鼠胰腺组织病理学、血清淀粉酶、IL - 6、IL - 8的变化 ,应用TUNEL法分析急性胰腺炎时胰腺腺泡细胞凋亡情况 ,WesternBlotting检测凋亡调控基因 -Bax的表达情况。 结果 :中药组大鼠血清淀粉酶 ,IL - 6、IL - 8水平较模型组低 ,病情明显改善 ,两组差异有显著性 (P <0 0 5 ) ;模型组凋亡指数为 2 34± 1 15 ,中药组为 11 5 2± 3 41,P <0 0 1;模型组Bax的表达水平为 96 8± 2 76 ,中药组为 1331± 112 ,P <0 0 1。 结论 :茵陈承气汤可能通过上调促凋亡基因Bax的表达水平 ,诱导已受损的腺泡细胞发生凋亡 ,减轻腺泡细胞坏死 ,减少胰酶及炎性介质的释放 。Objective:To investigate the effect of Yinchenchengqi decoction for the apoptosis of acinar cells and expression of Bax in severe acute pancreatitis models Methods:SD rats were divided at random into the control group,model group and herbal(Yinchenchengqi decoction)group.The severity and the serum level of amylase,IL-6 and IL-8 were observed at 6 hr after the injection of sodium deoxycholate.Apoptosis of acinar cells was analysed by TUNEL,the expression of Bax was examined by Western blotting.Results:The results showed that in the herbal group the serum levels of amylase,IL-6 and IL-8 decreased remarkably,the severity of pancreatitis were attenuated (P<0.05);apoptosis indexes of acinal cells in the treatment group were higher than those in the model group(11.52±3.41 v 2.34±1.15,P<0.01).The Bax expression level of acinar cells in the herbal group were higher than those in the control group(968±276 v 1331±112,P<0.01).Conclusion:It is suggested that Yinchenchengqi decoction might attenuate the severity of pancreatitis,reduce the release of amylase and inflammatory mediators through induced apoptosis of acinar cell possible involvement of Bax.
