摘  要:目的 :了解前列腺外科手术时不同高频功率引起的组织损伤情况。方法 :用狗建立前列腺电外科手术的动物模型 ,分别进行不同功率的电凝、电切、汽化 ,前列腺标本送病理检查 ,显微镜下测量组织损伤的厚度。结果 :电外科手术后 ,前列腺组织可见凝固性坏死 ,未见炎症反应。高功率电凝坏死层较厚 ,与低功率相比 ,两者有显著性差异 (P <0 .0 1) ;电切、汽化的坏死层厚度均与高频功率呈正相关 (P <0 .0 1) ;相同功率下 ,汽化和电切的坏死层厚度有显著性差异 (P <0 .0 1) ,汽化的坏死层更厚。结论 :相同功率下 ,前列腺汽化电切产生更厚的凝固坏死层 ,汽化的止血效果更好 ,在接近前列腺包膜和尖部 ,应选择低功率电切 。Objective: To investigate histological damage on prostate in electrosurgery with different high frequency power. Methods: Electrosurgical animal model was established in the dogs. The prostate was operated with electric coagulation,electrosurgical resection and electrosurgical vaporization using different high frequency power, The specimens were pathologically checked, the thickness of coagulation necrosis was determined. Results: Coagulation necrosis appeared in the prostatic tissue. In electric coagulation, the larger the power, the thicker the necrosis; in electrosurgical resection and electrosurgical vaporization, the thickness of coagulation necrosis was directly related to high frequency power. With equal power, the thickness of the necrosis of the vaporization was thicker than that of electrosurgical resection. Conclusion: Coagulation hemostasis of electrosurgical vaporization presented better results. Low power electrosurgical resection should be chosen in the prostate adjacent to envelope and apex.
