摘  要:目的 探讨三维彩色多普勒超声成像在移植肾术后监测中的图像特征及临床价值。方法 同种异体移植肾患者 10例。仪器为彩色多普勒超声诊断仪和三维彩色多普勒图像处理工作站 ,采用磁场空间定位自由扫查系统进行三维图像获取 ,脱机后进行肾脏结构及血流的三维图像重建与显示。结果  9例患者显示正常的移植肾三维图像 ,1例患者的移植肾三维重建图像显示异常。正常移植肾的血流呈“珊瑚”状 ,立体分布于整个肾实质 ,彩色血流信号均匀、对称、完整 ;血管栓塞时 ,栓塞血管的供血区域血流信号完全消失。结论 三维彩色多普勒超声成像新技术较传统二维彩色多普勒超声成像具有信息量更丰富 ,动、静脉血流显示更完整、直观、立体感强且灵活多样等优点 ,值得更深入地研究。Objective To evaluate the clinical value of three-dimensional (3D) color Doppler ultrasonography to monitor transplanted kidney after surgery and to display the characters of 3D imaging.Methods Ten patients who received allogenic transplantation of kidney were carried out with 3D color Doppler ultrasonography.The used instruments included color Doppler ultrasound diagnosis system and computer station of 3D color Doppler ultrasound imaging.The way of acquiring images was magnetic positioning free-hand scanning.3D reconstruction and display of renal structure of tissue and flow were performed off-line.Results Among 10 examined patients,9 transplanted kidneys were normal and 1 was of vascular embolism.The vasculature of normal transplanted kidney distributed among the entire renal substance in form of coral,and the color flow signals were of uniformity,symmetry and completeness.In the case of vascular embolism,color flow signals of the kidney which was provided with blood by the embolism renal artery disappeared completely.Conclusions In comparison with the traditional two-dimensional color Doppler ultrasonography,the 3D color Doppler ultrasonography has a certain superiorities containing much more information,more stereoscopic effect.It is more integrated and more flexible,which is worth further studying.
