摘  要:目的 观察联合应用生长激素和附加谷氨酰胺肠外营养对小肠黏膜上皮细胞分裂增殖能力的影响。方法 选用 40只手术成功的SD短肠大鼠 ,按 2× 2析因实验设计随机分为 4组 ,分别给予常规全肠外营养 (STD组 ,n=1 0 )、附加谷氨酰胺 (Gln组 ,n =1 0 )、附加生长激素 (GH组 ,n =1 0 )及附加谷氨酰胺和生长激素全肠外营养 (GG组 ,n =1 0 ) ,持续 6d。取 8只正常大鼠模拟手术后第 1天处死 ,作为基础对照组 (Control组 ,n =8)。应用图像分析和免疫组化技术对比观察大鼠小肠黏膜上皮细胞的DNA倍体分布和增殖细胞核抗原 (PCNA)表达。结果 单独应用生长激素或附加谷氨酰胺肠外营养均可不同程度提高小肠黏膜上皮细胞的DNA含量、S期细胞数及PCNA的表达 ,但两者联用的效果明显优于其单独使用的效果 (P <0 .0 1 )。结论 联合应用生长激素和附加谷氨酰胺肠外营养能够明显促进小肠黏膜上皮细胞的分裂增殖 。Purpose: To explore the effect of combination treatment with growth hormone(GH) and glutamine(Gln) enriched total parenteral nutrition(PN) on intestinal mucosal epithelial cell proliferation. Methods: Forty Spragur-Dawley rats with small bowel syndrome were randomly divided into four groups according to 2 x 2 factorial experiment and were treated with standard PN(S-PN group, n = 10), PN + Gln (Gln group, n = 10), PN + GH(GH group, n = 10) and PN + Gln + GH(GG group, n = 10) respectively for six days, Eight normal sham operation rats served as baseline controls(control group, n = 8). DNA content and expression of PCNA of intestinal mucosal epithelial cells were contrastly studied among those groups by using image analysis and immunohistochemical technique. Results: Gln or GH administration significantly increased intestinal mucosal epithelial cells DNA content, percentage of S-phase and expression of PCNA, But combined Gln and GH had better effect than Gln or GH administration individually (P<0.01). Conclusions: Combination treatment with growth hormone and glutamine enriched total parenteral nutrition can promote intestinal mucosal epithelial cell proliferation to prevent intestinal mucosal atrophy.
