充血性心力衰竭患者血清尿酸浓度的变化及与高血压的关系

摘　　要：目的　观察充血性心力衰竭 (CHF)患者血清尿酸浓度的变化规律及与高血压的关系。方法　比较 CHF患者(77例 )与心功能代偿的心血管病患者 (5 4例 )的血清尿酸浓度 ;比较 CHF患者不同心功能状态下尿酸浓度 ;比较入院时用利尿剂及未用利尿剂患者的尿酸浓度 ;比较 18例 CHF患者心力衰竭控制前后的尿酸浓度 ;比较 CHF组中有高血压者 (30例 )与血压正常者 (4 7例 )的尿酸浓度。结果　 CHF组与心功能代偿组的血清尿酸浓度分别为(32 7.72± 98.5 7)和 (2 30 .6 4± 5 3.14) mmol/ L;CHF组心功能按 NYHA分级 、 、 级的尿酸浓度分别为(2 5 0 .2 6± 6 7.2 9)、(314.99± 77.6 0 )和 (4 0 9.0 6± 78.15 ) mm ol/ L;入院时用利尿剂及未用利尿剂患者的尿酸浓度无明显差异 (P>0 .0 5 ) ;18例 CHF患者心力衰竭控制前后的尿酸浓度分别为 [(34 3.71± 10 3.32 )和 (2 6 9.42± 87.6 5 )mm ol/ L (P<0 .0 5 ) ];CHF组中有高血压者与血压正常者的尿酸浓度分别为 (36 5 .74± 110 .6 6 )和 (30 3.45± 88.10 )mm ol/ L。结论　 CHF患者血清尿酸浓度增高 ,且随着心力衰竭加重而逐渐升高 ;Objective To observe the changes of the uric acid level in patients with congestive heart failure (CHF) and the different changes in CHF patients with hypertension and in normotensive ones. Method The studied population consisted of 77 patients with CHF and 54 patients with cardiovascular disease whose heart function are normal as controls. We assess serum uric acid level in the following groups:①77 patients with CHF and 54 controls;②77 patients with different severity of heart failure;③Patients with and without diuretic therapy; ④Uric acid level of 18 CHF patients before and after the improvement of heart failure;⑤30 CHF patients with hypertension and 47 normotensive ones. Results The concentration of serum uric acid in CHF patients and controls was(327.72±98.57)and(230.64±53.14)mmol/L respectively;The concentration of serum uric acid in patients NYHA function class Ⅱ?Ⅲ?Ⅳ was(250.26±67.29)?(314.99±77.60)and(409.06±78.15)mmol/L respectively;There was no difference between the patients who received diuretics and not;Serum uric acid level was (343.71±103.32)and(269.42±87.65)mmol/L before and after improvement of heart failure(P<0.05);It was much higher in patients with hypertension than in normotensive subjects(365.74±110.66 vs. 303.45±88.10,P<0.05).Conclusion The serum uric acid level increase in patients with CHF. It is associated with deterioration of heart failure. Hypertension influences serum uric acid level. Patients with hypertension have a higher uric acid level.
