摘  要:目的 探讨外周血中的自由基反应在梗阻性黄疸 (以下简称阻黄或 OJ)及并发全身炎症反应综合征 (SIRS)发病中的作用。方法  4 0例阻黄患者按胆红素水平、梗阻的良恶性以及有无并发 SIRS分别分组 ,另取 30例健康正常人作为对照组 ,检测外周静脉血 -氧化氮 (NO)、维生素 C(Vit C) ,维生素 E(Vit E)、还原型谷胱甘肽 (GSH)的含量和超氧化物歧化酶(SOD)的活性变化。结果 阻黄患者组与正常对照组比较 ,NO含量明显增加 ,而 Vit C、Vit E,GSH的含量及 SOD的活性明显减少 (P<0 .0 1) ;有 SIRS的阻黄组与无 SIRS的阻黄组良性梗阻比较得出相同的结果 (P<0 .0 0 1) ,且胆红素浓度对各检测值有明显的影响 ,而良性梗阻与恶性梗阻各检测差异无显著性。结论 体内的自由基释放清除机制和氧化 -抗氧化机制紊乱可能在阻黄及并发 SIRS的发病中起重要。自由基的检测可能对阻黄并发Objective To explore the changes of plasma free radicals in patients with OJ complicted SIRS and to explore the relation between free radicals and severity of SIRS.Methods 40 case of OJ and 30 controls were studied.The concentration of plasma nitric oxide(NO),plasma VitC,plasma VitE,whole blood glutathione(GSH) and the activity of erythrocyte superoxide dismutase(SOD) were determind by spectrophotometry.Results Compared with the control group,the the average value of NO of the OJ group was significantly increased,and the average values of VitC,VitE,GSH and the activities of SOD were obviously decreased(P<0.01).The same results were observed in the comparison between the Oj with SIRS group and the OJ without SIRS group.The average values of NO,VitC.VitE,GSH,SOD were influenced by the degree of obstructive jaundice,but have no remarkable difference between the malignant OJ and benign OJ. Conclusion Disturbance of free radicals release clearance mechanism and oxidation antioxidation mechanism might have and important effect on the pathogenesis of OJ and its complication of SIRS.The NO and other free radicals may be used to determine and monitor the severity of patients with OJ and conplicated SIRS.
