针刺对准分子激光角膜切削术患者泪液中前列腺素E_2含量的影响

摘　　要：目的　评估针剌对免疫反应的作用。方法　 40例行准分子激光角膜切削术 (pho torefractivekeratectomy ,PRK)的患者用随机数字表法分为针刺治疗组 2 0例和未接受针剌治疗的对照组 2 0例。应用放射免疫测定法对两组患者术前及术后 1、 3、 7、 1 4、 3 0和 60天术眼泪液中的前列腺素E2 (prostaglandinE2 ,PGE2 )含量进行测定。结果　对照组PGE2 含量显著增加 ,术后第1 4天达最高水平 ,为 2 5 3 0 μg/L ,比术前平均水平高 3 5倍 ,随后逐渐下降 ,但高于术前水平(P <0 0 5) ;针刺组PGE2 含量亦显著增加 ,但低于同期的对照组 (P <0 0 5) ,随时间逐渐下降至正常水平。结论　PRK能诱导患者PGE2 的释放 ,进而导致免疫反应 ,术后PGE2 含量的变化可能是手术炎症与免疫应答的综合结果。针刺可以降低PRK术后PGE2 含量。提示术后早期应用针刺疗法有利于减少PGE2 的释放 ,抑制病理性免疫反应发生。OBJECTIVE To assess effect of acupuncture on immune reaction.METHODS Forty patients underwent eximer laser photorefractive keratectomy(PRK) were divided random by number table into treated group(20 cases) with acupuncture after PRK, and control group(20 cases) without acupuncture after PRK. To detect content of prostaglandin E 2(PGE 2) in tears of two groups before and 1, 2,7, 14, 30 and 60 days after PRK radioimmunoassay was used. RESULTS The content of PGE 2 after PRK was obviously high; there was a highest at 14 days postoperative content, i.e. as 25.30μg/L, it was 3.5 times as high as mean content before PRK. Then the content declined gradually, but it was higher than preoperative the content (P<0.05). The level of tears PGE 2 in treated group showed a significant increase, but was markedly less than the control group in the same time after PRK(P<0.05), and it declined gradually to normal level with the times. CONCLUSION The PRK could induce release of PGE 2 and further create the immune reaction. The postoperative early variation of PGE 2 could be complex effect of postoperative inflammation andimmune response. Acupuncture could reduce the PGE 2 content. This result showed that acupuncture was applied early after PRK to reduce release of PGE 2, and it could inhibit the immune reaction after PRK
