特发性膜性肾病肾组织中α-平滑肌肌动蛋白的表达及其意义

摘　　要：目的 :研究特发性膜性肾病 (IMN)肾组织内α -平滑肌肌动蛋白 (α -SMA)的表达及意义。方法 :对 15例IMN肾活检组织进行α -SMA免疫组化染色 ,染色结果半定量评分 ,并与相应肾小球及间质损伤程度比较。结果 :各期IMN肾小球及间质α -SMA表达均增高 ,并分别与肾小球损伤程度正相关 (r =0 .6 2 8,P <0 .0 5 ) ,与间质损伤程度显著正相关 (r =0 .932 ,P <0 .0 1)。结论 :肾组织固有细胞Objective:The expression and pathalogical significance of α-smooth muscle actin(α-SMA) expressed in renal tissues with idiopathic membranous nephropathy was studied.Methods:15 cases of renal biopsy specimens were immuno-histochemically stained with anti-α-SMA monoclonal antibodies.Results of immunohistochemical reactions were evaluated by semiquantitative method, and then compared with the glomerular and tubulointerstitial lesion grading. Results:The expression of α-SMA in the glomerular was increased, and related with the lesion grading of glomeruli (r=0.628,P=0.003). Significant positive correlations were found between the expression of α-SMA in renal interstitial tissues and the tubulointerstitial lesion grading(r=0.932,P=0.000). Some of tubular cells were found expressing α-SMA in the patients with worse lesion.Conclusion:An enhanced expression of α-SMA in renal tissues is a marker of the activation of these cells and reflects the glomerular and tubulo-interstitial lesion grading in certain degree. The transdifferentiation of renal tubulo-interstitial cells suggests that tubular cells maybe participate in the course of interstitial fibrosis.
