韧粘素在IgA肾病患儿肾组织中的表达及其意义

摘　　要：目的:研究IgA肾病(IgAN)儿童肾组织中韧粘素(TN)的分布、表达以及其与疾病活动性和慢性化指标的关系。方法:采用免疫组化ABC法,观察12例IgAN肾组织中TN蛋白分布及表达。结果:①TN在正常肾组织中,肾小球阴性、肾小管间质及血管(动脉)局灶性弱阳性表达。而在IgAN病人主要于ECM积聚处表达明显增加,且其在肾小球表达与肾小管间质表达正相关(r=0.675,P<0.05)。②TN表达与IgAN病人慢性化指标正相关(肾小球:r=0.572,P<0.05;肾小管间质:r=0.824,P<0.01),而与活动性指标关系不密切(P>0.05)。③TN在肾组织中表达与Scr、尿蛋白排泄量相关,而与性别、年龄、病程、血清总蛋白、白蛋白等临床指标无关(P>0.05)。结论:①TN在IgAN病人肾组织中ECM积聚处表达明显增加。②TN蛋白水平在肾小球及肾小管间质表达可作为IgAN慢性化指标。为临床判断IgAN病人病情轻重、预后及治疗提供依据。Objective:To evaluate the expression of tenascin(TN)of renal tissu es in children with IgAN and its relationship among TN expression,disease activ ity and chronicity in those patients.Methods:The TN expression in 12 renal spec imens of IgAN patients were investigated by the immunohistochemical method;the activity and chronicity index in those patients were also evaluated. Results:① TN expression was significantly increased in IgAN,and its expression in the glo meruli was correlated with that in the tubulointerstitium (r=0.675,P< 0.05).②T N expression of renal tissues was correlated with the chronicity index in those patients (glomeruli:r=0.572,P< 0.05;tubulointerstitium:r=0.842,P< 0.01).③T here was a significantly positive correlation among the expression of TN,Scr an d urinary protein loss in IgAN patients.Conclusion:①TN expression was signific antly increased in IgAN patients.②TN expression may serve as the chronicity ind ex in IgAN.
