脚池的显微解剖学研究

摘　　要：目的 :探讨脚池的显微解剖特点及其临床意义。方法 :在手术显微镜下 ,观测 15例尸颅脚池的构成及其结构特点。结果 :脚池位于大脑脚与颞叶之间 ,居于环池的前面及上面 ,仅见于半数标本。脚池的底壁呈网膜样 ,多数自钩向内上方伸展 ,前后长度为 6 .0～ 12 .0mm。该壁越向后越窄 ,在部分标本明确分隔脉络膜前动脉和后交通动脉。脚池的内侧壁为大脑脚 ,顶壁为视交叉、视束及前穿质 ,外侧壁为颞叶内侧面 ,向前开放于颈动脉池。结论 :脚池不是脚间池向后外侧方的延伸 ,二者相互独立 ,池内也并不含有内侧脉络膜后动脉。由于脚池底膜变异大 ,难以成为可靠的标志。手术中达到后交通动脉池时 ,若在外上面见一层网膜样结构 ,不要轻易打开 。Objective:To determine the microanatomical characteristics and clinical significance of crural cistern. Method:The composition and structural characteristics of rural cisterns were observed in 15 cadeveric heads under an operating microscope.Results:Crural cistern located between cerebral peduncle and temporal lobe.It was anterior and superior to ambient cistern and opened into carotid cistern anteriorly.It was only found in half of the observed secimens.The basal wall was a sheet of arachnid with many little meshes.This memberane stretched medial-superiorly from uncus.This wall got narrower and narrower posteriorly.This membrane clearly separated anterior choroidal artery (AChA) from posterior communicating artery (PCoA) in some specimens.The roof wall was an inferior surface of optic chiasma/tract and anterior perforated substance.Lateral wall was a medial surface of temporal lobe.Conclusions:Crural cistern is not a posterior-lateral extension of interpeduncular cistern.Medial posterior choroidal artery is not contained in crural cistern at all.The basal wall can not be used as a dependable mark in the operation,as it varies greatly.If a sheet of arachnoid structure is found to be lateral-superior to PCoA in the operation of the cistern of PCoA,this membrane should not be opened rashly,because there is almost always AChA cursing lateral-superior to this membrane.
