HLA配型在心脏移植中的意义

摘　　要：目的 :探讨人类白细胞抗 (HLA)配型在心脏移植中的临床意义。方法 :采用聚合酶链反应 -序列特异性引物 (PCR -SSP)技术进行供受者外周血HLA基因分型 ,采用单克隆抗体法进行HLA -Ⅰ类抗原分型。结果 :供受体之间HLA有 6个抗原相合 :A2 4 (9) ,Bw6 ,DR1 5(2 ) ,DRw51 ,DRw52和DQ6(1 ) ,有 5个抗原相合 :A30 (2 9) ,B54(2 2 ) ,Bw4 ,DR1 7(3)和DQ2 ,术后患者心功能正常 ,术后 1 60d无排斥反应。结论 :良好的HLA配型对减少心脏移植排斥反应 ,提高移植物存活率具有重要意义。Objective:To elucidate the clinical significance of human leukocyte antigen (HLA) matching in donors and recipients of heart transplantation. Method:Donor and recipient HLA class Ⅱ gene typing was performed by PCR amplification with sequence specific primers (PCR-SSP) an HLA class Ⅰ typing with special monoclonal tray.Results:There were 6 HLA antigens matching and 5 HLA antigens mismatching between donor and recipient.The patient was recovery without acute rejection.The heart function.The heart function was normal.Conclusions:Good HLA matching plays an important role in reducing the incidence of acute rejection and in improving the survival of heart transplants.
