安定颈动脉内注射对癫癎灶定侧的价值

摘　　要：目的 :探讨颈动脉内注射安定对癫灶定侧的价值。方法 :对 12例 (10例两侧同步和 2例游走性样放电 )癫患者进行安定颈动脉内注射试验 ,观察注射后样波受抑制情况来确定灶侧。结果 :于灶侧安定颈动脉内注射 ,两侧样波同时消失 ;于灶对侧注射 ,则注射侧样波消失 ,而病灶侧样波仍存在。结论 :颈动脉内注射安定对癫灶定侧有重要价值 ,可替代传统的颈动脉内阿米妥钠注射试验。Objective:To explore the value of injecting diazepam into the carotid artery in lateralization of epileptic focus.Methods: A test of injecting diazepam into the carotid artery was done in ten patients with bilateral synchronized epileptiform discharges and two ones with itierant epileptiform discharges.The focus side was determined by observing the inhibition of epileptiform discharges after injecting diazepam.Results:Bilateral epileptiform discharges disappeared simultaneously as diazepam was injected into the ipsilateral carotid artery of the epileptic focus side.When diazepam was injected into the contralateral carotid artery of the focus side.The epileptiform discharges of the injection side disappeared,however,the epileptiform discharges of the focus side still existed.Conclusions:Injection of diazepam into the carotid artery might be of important value in lateralization of the epileptic focus.It might take place of the traditional test of injection of sodium amytal into the carotid artery.
