羟基磷灰石在兔颈椎双开门手术中的应用

摘  要:目的 :以羟基磷灰石 (Hydroxyapatite ,HA)代替松质骨用于兔棘突纵切式双开门手术 ,探讨其临床应用的可能性。方法 :将 2 2只日本大耳兔分为双开门组和单开门对照组 ,所用羟基磷灰石呈长方体。术后 3月 ,手术节段行大体解剖、X线、光镜及扫描电镜观察。结果 :双开门组存活良好 ,羟基磷灰石与骨紧密接触愈合 ,椎管内无骨痂及瘢痕组织生长 ,脊髓无炎性反应。结论 :羟基磷灰石用于棘突纵切式双开门是安全的 ;能避免单开门手术后的再关门及椎管内瘢痕的长入 ;Objective:To investigate the bioreaction of cervical spine to hydroxyapatite (HA) spacer which had been inserted between the two halves of a splitted process in a rabbit model. Method:Twenty two adult rabbits had been used,twelve(group A)received spinous process splitting laminoplasty with HA spacer;ten(group B)received unilateral open door laminaplasty.The shape of this HA spacer is cuboid.General observation,X ray,HE histologic section & SEM were done of the operated parts at three months after operation.Results:All animals of group A had survived in 3 months.Implants of group A directly bonded to bone without any fibrous tissue in the spinal canal,there is more or less scar adhering to the spinal cord in group B( P <0.05),no inflammation signs have been found in the spinal cord of both groups.SEM shows direct contact between the newly formed bone and HA implants.Conclusion:It is safe to use HA spacer in spinous process splitting lamino plasty instead of autogenous bone.This procedure prevents scar migrating into the spinal canal and the reclossing which may occur after unilateral open door laminoplasty,and spinous process splitting laminoplasty will be simplified.
