三维彩色多普勒超声成像在诊断颈动脉粥样硬化中的应用研究

摘　　要：目的　探讨颈动脉粥样硬化的三维超声成像特征及其临床价值。方法　健康志愿者 3 2例 ;颈动脉粥样硬化患者 3 0例 ,均经二维超声证实有 1处以上的粥样斑块形成。应用彩色多普勒超声诊断仪及三维彩色多普勒成像系统 ,三维图像采集方法选用自由臂方式 ,扫描过程包括血管长轴扇形扫描和血管短轴平行扫描。脱机进行血管壁、管腔及血流的三维重建与显示。结果　正常颈动脉的三维重建图像能够连续、完整地显示出颈总动脉、颈内动脉、颈外动脉的空间走行 ,可见其管壁内膜面光滑平整 ,管腔通畅 ,血流充填完满。粥样斑块形成的颈动脉三维重建图像可直观显示斑块的立体形状、表面特征、确切部位、血管腔大小及血流的空间走行情况。Objective To probe into characters and clinical value of three dimensional color Doppler imaging on carotid atherosclerosis.Methods Thirty two healthy volunteers and 30 patients with carotid atherosclerosis who were confirmed by two dimensional ultrasonography to have one or more atheromatous plagues were examined.The used instruments included Acuson 128XP color Doppler ultrasonography diagnosis system with L5/7 MHz linear array transducer and TomTec three dimensional color Doppler ultrasonography system(Echoscan 4.2 ).The free hand mode of acquiring imaging was employed.The process of scanning included sector scanning of arterial long axis and parallel scanning of arterial short axis.The three dimensional reconstructions and of displays of arterial wall,arterial vessel and blood flow were finished off line.Results The three dimensional reconstruction images of carotid artery can display the continuous and entire stereo form of common carotid artery,internal carotid artery and extral carotid artery.Three dimensional images of normal carotid artery showed smooth endarterium,unobstructed vessel,complete filling of blood.The three dimensional images of carotid artery with atheromatous plaque showed directly the stereo shape,surface character and exact position of plague,volume of vessel,and spatial distribution of blood flow.Conclusions The three dimensional color Doppler ultrasonography has important clinical value and application potentialities for imaging diagnosis of carotid atherosclerosis.
